摘要
目的观察女性生殖器尖锐湿疣(CA)复发与不复发患者的外周血Th1/Th2细胞因子水平和病程情况,探讨其复发与其外周血Th1/Th2细胞因子和病程的相关性。方法选取女性生殖器CA初发患者37例,经电灼治疗后随访6个月(期间不使用任何免疫制剂)是否复发分为两组:再次复发的为A组15例,不再复发的为B组22例。用ELISA法测定电灼治疗前各血清样品的细胞因子IL-2、IL-12、IL-4、IL-10的浓度,比较A、B两组血清各细胞因子及Th1/Th2比值和病程的差异,分析CA复发与血清Th1/Th2细胞因子水平和病程的相关性。结果CA复发与Th1/Th2比值呈显著负相关,与发病病程呈显著正相关;经ROC分析,Th1/Th2比值来预测CA复发有较高的敏感性(86.4%)和特异性(80.0%),ROC曲线下面积为0.812;而临床指标发病病程来预测CA复发的敏感性、特异性分别为53.3%、90.9%,ROC曲线下面积为0.732。结论CA复发与否与患者血清Th1/Th2细胞因子水平和发病病程密切相关,Th1/Th2比值和发病病程可作为评价CA是否易复发的参考指标。
Objective To investigate serum T helper type 1/T helper type 2（Th1/Th2）cytokine levels and its correlation with the recurrence of wart（codyloma acuminate,CA）after electric cautery treatment in female patients.Methods Thirty-seven patients with primary genital warts were treated with electric cautery and later had 6-month follow up without any immunological treatment.Fifteen participants had CA relapse and 22 participants had no recurrence of CA.Serum concentration of cytokines were tested with ELISA kits for interleukin（IL）-2,IL-12,IL-4 and IL-10.The production of cytokines and Th1/Th2 ratio were compared in the patients with CA relapse and in those without the recurrence of disease.Results The recurrence of CA was negatively correlated with Th1/Th2 ratio,but was positively correlated with the duration of CA.The sensitivity,specificity and the area under the ROC curve were 86.4%,80.0% and 0.812 respectively in predicting the CA relapse with Th1/Th2 ratio;while which were 53.3%,90.9% and 0.732 respectively in predicting CA relapse with CA duration and other clinical indicators.Conclusion The results suggested that CA relapse was associated with both the ratio of Th1/Th2 cytokines and the duration of CA,which could be valuable in predicting the recurrence of CA after treatment.
