摘要
目的检测慢性阻塞性肺疾病(COPD)患者痰基质金属蛋白酶-9(MMPO)和基质金属蛋白酶抑制剂-1(TIMP-1)的变化,探讨COPD患者肺功能下降与MMPO、TIMP-1的关系。方法COPD急性加重期患者20例、稳定期患者12例和9例健康对照者提供了痰标本,同时进行肺功能测定。采用双夹心抗体酶联免疫吸附测定实验检测痰上清液MMPO、TIMP-1含量。结果痰上清液MMPO浓度COPD急性加重期组高于稳定期组和对照组(P〈0.01),稳定期组高于对照组(P〈0.05);痰上清液MMPO/~MP.1浓度COPD急性加重期组高于稳定期组和对照组(P〈0.001),稳定期组低于对照组(P〈0.05)。COPD急性加重期患者痰MMPO、MMPO/TIMP-1与第一秒用力呼气容积占预计值百分比(FEV,%pre)呈负相关(r=-0.573、P〈0.05,r=-0.625、P〈0.05)。结论COPD患者急性加重时痰MMP-9增加、MMPO/TIMP-1失衡,并且可能是肺功能下降的原因。
Objective To evaluate the level of sputum MMP-9,TIMP-1 in chronic obstructive pulmonary emphysema diseases (COPD) during exacerbation and discuss it relationship with lung function. Methods Sputum samples and lung function were provided from 20 COPD patients during exacerbation stage,12 during sationary stage and 9 healthy control,the concentration of MMP-9 and TIMP-1 was detec- ted in the supernate fluid of sputum samples with enzyme-linked immunosorbent assay(ELISA). Results ( 1 ) The MMP-9 concentration in the supernate fluid of sputum samples was increased(P 〈0.01 ) in COPD exacerbation and stationary stage. The MMP-9/TIMP-1 rations in the supernatant fluid of sputum samples was increased in COPD during exacerbation comparing to stationary stage and healthy control ( P 〈 0. 001 ) ;the MMP-9,MMP-9/TIMP-1 sputum ratios in COPD were negatively correlated with FEV1 % pre (r= -0.573,P 〈0.05,r = -0. 625,P 〈0.05). Conclusion MMP-9 and the imbalance of MMP-9/ TIMP-1 ratios may play a role in the decreased lung function in COPD during exacerbation.
