腓骨小头复合皮瓣重建足前掌部缺损的生物力学实验与临床研究 被引量:36

摘要 目的:本文通过7倒下肢标本,测试足前掌部外伤性缺损与重建后足的生物力学变化。方法:模拟第一跖骨头缺损,然后应用腓骨小头复合皮瓣重建足前掌部的缺损,进行生物力学实验。结果:此方法不仅能满足下肢强度、刚度和稳定性的要求,而且符合足生物力学原理。结论:其两者形态结构上的均符合生理要求,重建后构成了合理的足弓力学结构,而重建后的下肢的应力、应变、位移和轴向刚度上与原始标本无显著性差异(p>0.05),而与缺损下肢呈显著性差异(p<0.01),临床上36例足前掌部缺损重建后的功能良好。 Objective; To determine the biomechanical changes in the forefoot palm with traumatic defect before and after reconstruction. Methods: 7 specimens were used. Each specimen was experimentally tramatized to result in a defect in the first metatarsal head. Fibular head- compounded flap was employed to reconstruct the fore fbot palm with defect, and biomechanical determinations were carried out. Results: With this method, not only the requirements of strength, rigidity and stability of the lower limb were met, but the biomechanical principles were satisfied as well. Conclusion : The first metatarsal head and the fibular head, in terms of morphological structure, are both consistent with the physiological reguirements and the mechanical structure of the arch of the foot after reconstruction is also rational. The post- reconstruction stress, strain, shift and axial rigidity in the lower limb are not significantly different from the pre- trauma values (P>0.05), but significantly different from those values in the lower limb with defect(Pwt0.01). Clinically, 36 cases of forefoot palm with defect have been reconstructed in this way, and the functions after reconstruction are satisfactory.
