μ阿片受体A118G基因多态性对妇科手术患者疼痛敏感性的影响 被引量:8

Effects of A118G polymorphism in μ opioid receptor gene on pain sensitivity in patients undergoing gynecological operation

摘要 目的μ阿片受体A118G基因(OPRM1 A118G基因)多态性对妇科手术患者电刺激痛敏感性的影响。方法择期拟行子宫肌瘤剔除术或子宫全切术患者152例,汉族,年龄20~50岁,体重指数19~21kg/m^2,ASAⅠ或Ⅱ级,根据基因型分为野生型纯合子组(A/A组)、突变型杂合子组(A/G组)和突变型纯合子组(G/G组)。采用电刺激测定患者痛阈和耐痛阚。使用聚合酶链反应.限制性片段长度多态性技术检测OPRM1 A118G基因多态性。结果A/A组76例,A/G组56例,G/G组20例;G等位基因频率为31.6%。三组间痛阈差异无统计学意义(P〉0.05);G/G组耐痛阈低于A/A组和A/G组(P〈0.05);A/A组和A/G组耐痛阈差异无统计学意义(P〉0.05)。结论OPRM1 A118G基因突变可导致妇科手术患者耐痛阈降低,对痛阈无影响。 Objective To determine the effects of A118G polymorphism in μ opioid receptor gene on pain sensitivity in patients undergoing gynecological operation. Methods One hundred and fifty-two gynecological patients, aged 20-50 yr, ASA Ⅰ or Ⅱ , with body mass index 19-21 kg/m^2,Han nationality, were divided into 3 groups according to the genotype: group A/A (wild-type homozygous) ; group A/G (mutant heterozygous) and group G/G (mutant homozygous). Pain threshold and pain tolerance threshold were measured using electrical stimulation. Genotypes were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR- RFLP) for A118G polymorphism in μ opioid receptor gene. Results Genotyping analysis revealed that genotype distribution of A118G polymorphism was A/A 76 cases, A/G 56 cases and G/G 20 cases. The G allele frequency was 31.6%. There was no difference in pain threshold among the 3 groups (P 〉 0.05). But the pain tolerance threshold was significantly lower in group G/G than in group A/A and group A/G (P 〈 0.05). Conclusion The A118G allele gene mutation in μ opioid receptor gene decreases pain tolerance threshold but has no effects on pain threshold in patients undergoing gynecological operation.
