摘要
对Ⅱ、Ⅲ期乳腺癌６９例综合方法疗效对比分析。全组５年生存率５３６％，Ⅱ期７８％，Ⅲ期４７２％，Ⅱ期各组（Ｓ，Ｓ＋Ｃ，Ｓ＋Ｒ，Ｓ＋Ｒ＋Ｃ）无显著差别；Ⅲ期各组Ｓ１４２％，Ｓ＋Ｃ３３％，Ｓ＋Ｒ４７３％，Ｓ＋Ｒ＋Ｃ６０％（Ｐ＜００５）说明综合治疗方法对生存率有影响，增加放疗对Ⅲ期尤为显著；加放组淋巴区胸壁３年复发率与未加放组比为１∶２（９／５０：７／１９），其复发平均时间为３１８∶１３５（月），说明加放对局部控制效果的肯定。增加化疗（ＣＭＦ／ＣＡＦ／ＣＥＦ）与未加化疗比生存率有改变，前者为５９５％（２２／３７），后者为３３３％（３／９），而除Ｓ组外，增加化疗使Ｓ＋Ｒ＋Ｃ达６０％，Ｓ＋Ｒ４７３％组无显著性差别（０２５＞Ｐ＞０１０）。Ｓ＋（Ｓ＋Ｃ）与（Ｓ＋Ｒ）＋（Ｓ＋Ｒ＋Ｃ）组５年生存率，前者为３６８％（７／１９），后者为６０％（３０／５０）。
Objective:To compare 69 cases of breast cancer of stage Ⅱ and Ⅲ with single or combined therapy and to analyse the result.Materials and Methods: From Feb.1984 to Oct.1990,69 cases of stage Ⅱ and Ⅲ of breast cancer were recruited and divided into 4 groups:surgery only(S),surgery+chemotherapy (S+C),surgery+radiotherapy (S+R) and surgery+radiotherapy+chemotherapy(S+R+C).Operations were undertaken as:simple mastectomy,radical mastectomy and modified mastectomy.Radiotherapy was delivered with doses of Dr 45～55 Gy/28～30 d/5～6 wk to the chest wall and to ragional lymphatic areas.Chemotherapy schedule of CMF or CAF/CEF was given for 4～6 cycles postoperatively.Results:The overall 5-year survival rate was 53 6%,e.g.stage Ⅲ 78%,stage Ⅱ47 2%.The 5-year survival rates of four groups(S,S+C,S+R and S+R+C) of stage Ⅱ were no differences( P >0 05).The 5-year survival rates of stage Ⅲ were:S 14 2%(1/7),S+C 33%(2/6),S+R 47 3%(9/19) and S+R+C 60 8% (14/23).The difference between S group and the others was statistically significant ( P <0 005).Conclusion: (1)the combined therapy especially plus radiotherapy, improve the survival rate. (2)The Tumor recurrence rate in chest wall 3-year after the operation were 9/50(plus radiotherapy)and 7/19 (no plus radiotherapy).Postoperative radiotherapy could markedly decrease the local tumor recurrence.(3)Plus chemotherapy (CMF/CAF/CEF) could change the survival rate, [59 5%(with plus chemotherapy) and 33 3% without chemoterapy)]. (4) The S+(S+C)+(S+R) and S+R+C in stage Ⅲ of 5-year survival rate were 36 8%(7/19)and 60% (30/50). (5)The 5-year survival rate of stage Ⅲ of breast cancer could be enhanced by combined therapy.
