摘要
目的探讨截断腓骨在胫骨干骨折不愈合治疗过程中的作用。方法回顾性分析48例胫骨干骨折术后12～34个月骨不连患者临床资料，采用截断腓骨、扩髓交锁髓内钉内固定加植骨的手术方法治疗。结果所有患者均获14～30个月的随访（平均18个月），48例患者中47例获得了骨性愈合，1例患者由于不配合治疗进行剧烈运动从而导致再骨折。结论截断腓骨，解除因腓骨和髓内钉导致的应力遮挡，使骨折断端进行加压，增加断端接触面积，从而能够有效的促进骨折愈合。
Objective To explore the effects of cutting fibular to cure the nonunion of tihil shaft fracture. Methods 48 nonunion cases of tibil shaft fractures had been operated 12 to 34 months before,were treated by cutting fibular, intramedullary interlocking nailing and bone grafting,all the clinical data were retrospectively analyzed. Results All cases were followed-up from 14 months to 30 months （average 18 months）,47 cases had unnioned well. Only one case was refractured because he had not cooperated with doctor＇s treament and he done strenuous exercises. Conclusion Cutting fibular would dismiss stress protection from fibular and intramedullary interlocking nailing,make the fracture carrying out compression,and increase the fracture＇s touch acera which can advance the fracture union.
