摘要
目的总结心脏移植术后感染的特点，对其预防及治疗进行讨论。方法对北京安贞医院心脏外科１９９２年３月及１９９７年１１月施行的２例原位心脏移植手术进行总结，并结合有关文献对心脏移植术后感染的临床表现和治疗进行总结。结果１例术后发生５次感染，３次为呼吸系统细菌感染，２次病毒感染［１次为皮肤的带状疱疹感染，１次为支气管肺脏的人巨细胞病毒（ＨＣＭＶ）及ＥｐｓｔｅｉｎＢａｒ（ＥＢＶ）感染］。尸检证实死亡原因为急性排斥反应及感染造成的右侧支气管肺炎，存活７个月；另１例目前情况良好，已存活８个月，未发生感染。结论心脏移植围术期感染确有一定的发病特点。
Objective To observe the characteristics of infection after cardiac transplantation and discuss the prophylaxis and management of infection after heart transplantation. Method Two cases of orthotopical cardiac transplantation were performed. We discussed the clinical characteristics and management of infection after heart transplantation with a review of the literature. Result Infection was noted for 5 time in one case:infection in the respiratiory system caused by bacteria(3), and viral infection (2). Viral infection included zoster in skin (1) and infection of bronchus and lung by human cytomegalovirus(HCMV) and Epstein Barr virus(1). One patient died 7 months after heart transplantation. Autopsy showed that death was due to acute rejection and bronchopneumonia resulted from viral infection. Another patient has been surviving for 8 months. Conclusion Infection after heart transplantation has some characteristics,which are important for early diagnosis and managment.
