摘要
[目的]评价两种结核菌快速检测方法在口岸卫生检疫中的应用价值。[方法]以"中华人民共和国行业标准-肺结核诊断标准WS288-2008"选取结核病人100例,有呼吸道症状的疑似病人50例,以罗氏L-J固体培养结果为标准,考核荧光实时定量PCR(RT-PCR)、分枝杆菌直接检测(MDT)在结核病诊断的敏感性、特异性。[结果]100例结核病人标本培养后均为阳性,50例可疑者标本培养后均为阴性;100例培养阳性的标本,荧光实时定量PCR检测出98例阳性,MDT检测出100例阳性;50例培养阴性标本,荧光实时定量PCR检测出1例阳性,MDT未检测阳性。RT-PCR的敏感性、特异性、阳性预期值、阴性预期值为98.04%、98.04%、99.01%、96.15%;MTD检测敏感性、特异性、阳性预期值、阴性预期值均为100%。[结论]与传统培养方法相比较,两种方法有着很高的敏感性和特异性,但检测时间大大缩短,适合应用于口岸卫生检疫中。
To evaluate the value of two rapid methods for detecting the mycobacterium tuberculosis in frontier health quarantine.100 tuberculosis patients and 50 respiration symptom suspect patients were selected according to pulmonary tuberculosis diagnosis criterion,the sputum samples from the cases selected were detected using the two rapid methods,real time PCR(RT-PCR)and Gen-probe mycobacterium tuberculosis direct test(MDT),and L-J medium cultured whose result is regarded as standard.The culture results from 100 tuberculosis patients are all positive and from 50 suspects are all negative.98 and 100 positive results were respectively obtained from using RT-PCR and MDT to detect 100 culture positive sputum samples.49 and 50 negative results were obtained from 50 culture negative sputum in the same way.RT-PCR and MDT sensitivity,specificity,positive predictive value and negative predictive value were 98.04%,98.04%,99.01%,96.15% and 100%,100%,100%,100%,respectively.Comparing with the traditional culture method,the two rapid mycobacterium tuberculosis detections show high sensitivity and specificity,short time operation duration,and are suitable for frontier health quarantine.
