摘要
目的:评价智能超声定量(AQi)和彩色动力图(CK)新技术探测心肌梗死(MI)患者左室功能和室壁运动的临床应用价值。方法:MI组46例,对照组30例。使用配备有AQi、CK装置的HP2500型的彩超仪。AQi探测取心尖四腔心切面,以容积变化率曲线和数字形式迅速显现出左室舒张末期容积(EDV)、左室收缩末期容积(ESV)、射血分数(EF)和峰值充盈率(PFR)、峰值充盈率时间(TPFR)。CK在AQi基础上显示出左室心内膜全收缩期和全舒张期的彩色变化和位移。结果:与对照组相比,AQi示MI组的EDV和ESV明显增大,心腔形态不规则,EF值变小,PFR降低,TPFR延长,即左室功能下降。同时,CK显示MI组梗死部位的心内膜在收缩或舒张期的颜色变化与对照组不同,彩色位移明显减少。结论:AQi、CK技术在探测心脏形态、大小、室壁运动和定量评估心脏功能方面为临床提供了准确可靠的诊断依据。
Objective: To judge the clinical practical value of the intelligent automated quantification (AQi) and color kinesis (CK) for detecting the left ventricular(LV) function and wall movement of the patients with myocardial infarction(MI). Methods: Fortysix patients with MI and thirty normal subjects were measured by HewlettPackard Sonos 2500 ultrasound system with AQi and CK installation. AQi displayed automatically as waveforms and digital information on apital fourchamber view. The displaying contents included enddiastolic volume (EDV), endsystolic volume (ESV),ejection fraction (EF), peak filling rate (PFR), time to peak filling rate(TPFR),etc. CK imagings obtained on the basis of AQi technique showed the color change and displacement of the LV endocardium in pansystole or pandiastole. Results: The EDV and ESV of the patients with MI were more enlargement than control group. The LV chamber shape was irregular,EF was reduced, PFR decreased and TPFR prolonged. These results indicated that LV function dropped in patients with MI. In the meantime, we found that the color change of the regional endocardium of infarct wall was different from normal ones and color displacement lessened remarkably. Conclusion: This study shows that AQi and CK new techniques are able to offer accurate and reliable diagnosis basis for detecting LV shape,size,wall movement and function.
