糖尿病周围神经病变相关危险因素分析 被引量:5

Analysis on Risk Factors for Diabetic Peripheral Neuropathy

摘要 目的探讨糖尿病周围神经病变(DPN)的相关危险因素。方法收集349例2型糖尿病住院患者的年龄、糖尿病病程、体质指数(BMI)、血压、冠心病史、空腹血糖(FPG)、空腹胰岛素(FINS)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)小血脂、胰岛素抵抗指数、24h尿蛋白定量等资料。根据患者神经系统的症状、体征、简易神经病变功能障碍评分、神经电生理检查,将患者分为DPN组(203例)和非DPN组(146例)。对各项指标进行单因素Logistic回归分析,对有统计学意义的因素行多元Logistic回归分析。结果年龄、糖尿病病程、BMI、高血压、血糖控制差、患有冠心病、空腹血糖高、2hPG是DPN的危险因素。多元Logistic回归分析显示,病程(OR=1.075,95%CI=1.164~7.379,P=0.022)、2hPG(OR=0.135,95%CI=1.030~1.318,P=0.015)是DPN的独立危险因素。结论DPN的发生与患者糖尿病病程和餐后血糖水平增高有关。糖尿病病程、2hPG是DPN的独立危险因素。 Objective To study the risk factors for diabetic peripheral neuropathy (DPN). Methods The clinical data of 349 patients with type 2 diabetes, including the age, course of diabetes, body mass index (BMI), blood pressure, Coronary heart disease (CHD), fasting plasma glucose (FPG), fasting plasma insulin (FINS), 2 h postprandial plasma glucose (2h PG), HbA1c, lipidss, Homa-IR, protein quantitation of the 24 hours urine, etc., were collected. The patients were divided into 2 groups according to their symptoms of nervous system, signs, neuropathy dysfunction score, and examination of neural electrophysiology: DPN group (203 cases) and non-DPN group (146 cases). The factors were subjected to univariate logistic regression analysis; the factors with statistical significance were subjected to multivariate logistic regression analysis to screen for the risk factors of DPN. Results The ages, courses of diabetes, pressures, BMI, CHD, HbA1c, FBG, OGTT 2h PG were the risk factors of DPN in univariate logistic regression analysis. Multivariate logistic regression analysis revealed that DPN was correlated with patient's course of diabetes (OR=1.075, 95%CI: 1.164-1.7.379) and 2h PG (OR=0.135, 95%CI: 1.030-1.318). Conclusion DPN is associated with courses of diabetes and 2h PG. Courses of diabetes and 2h PG were the independent risk factors.
