妊娠期高血压疾病的血液动力学分析 被引量:4

摘要 目的:探讨妊娠期高血压疾病(PIH)患者病情程度与血液动力学改变的关系。方法:用MP妊高症监测仪检测妊娠期高血压疾病患者血液动力学指标,并分析其与妊娠期高血压疾病患者病情程度的关系。结果:①PIH组平均动脉压(MAP)、动脉压差(DP)、外周阻力(TPR)、血液粘度(V)较正常妊娠组明显升高,差异有统计学意义(P<0.01);②MAP、DP、TPR、V等指标在妊娠高血压组即有升高,在轻度子痫前期、重度子痫前期时,上述指标均有显著改变,且随病情加重而更趋于明显。重度子痫前期组每搏输出量(SV)、心输出量(CO)、血管顺应性(AC)较妊娠高血压组明显下降,差异有统计学意义(P<0.05);③妊娠期高血压疾病孕妇血液动力学类型主要包括:低排高阻型(41.33%)、正常排高阻型(36%)、正常阻力型(12%)、高排低阻型(8.0%)、正常排低阻型(2.67%)。结论:妊娠期高血压疾病孕妇血液动力学类型以低排高阻型、正常排高阻型为主,检测妊娠期高血压疾病孕妇血液动力学指标对判断妊娠期高血压疾病的严重程度有重要价值。 Objective: To investigate the relationship of hemodynamics changes and the severity of the patients with pregnancy induced hypertension. Methods: By MP monitoring system to detect hemodynamics parameters in the pregnancy induced hypertension, and analyze the relationship of hemodynamics characteristics and the severity of the pregnancy - induced hypertension. Results: (1)Mean arterial pressure (MAP) , arterial pressure (DP) , total peripheral resistance ( TPR ) , blood viscosity (V) of the PHI patients were higher than those of normal pregnant women (P 〈 0.01 ); (2) Mean arterial pressure (MAP), arterial pressure (DP), total peripheral resistance (TPR), blood viscosity (V) of the gestational hypertension were higher than the normotensive group, and with the severity of hypertensive disorders in pregnancy increase by degrees; (3)The PIH patients hemodynamics was mainly divided into following types: low CI with high TPR (41.33%), normal CI with high TPR (36%), normal TPR ( 12% ) , high CI with low TPR (8. 0% ), and normal CI with low TPR (2. 67% ) . Conclusion: The PIH patients have different hemodynamic profiles and mainly divided into types: low CI with high TPR and normal CI with high TPR. The prediction of PIH from the changes in hemodynamic parameters has significant implications for clinical practice.
