摘要
目的应用二维应变技术分别测量左室心肌缺血时的心肌运动速度(Vel)和二维应变(Str),并对比两种技术相关指标在病理状态下的变化趋势。方法18例缺血性心肌病左室射血分数降低患者[EF<50%,(36±11)%]及20例正常对照志愿者[EF≥50%,(63±17)%]纳入本研究,应用斑点追踪技术二维应变(STE)分析软件分别测量左室长轴方向各室壁的左室收缩期心肌运动速度(S-Vel)、舒张期心肌运动速度(D-Vel)、收缩期应变(S-str)、舒张期应变(D-str),并对两组测量结果进行对比分析。结果心肌缺血组左室各游离壁各指标包括收缩期峰值速度(S-Vel)、舒张期峰值速度(D-Vel)、收缩期峰值应变(S-str)、舒张期峰值应变(D-str)均较正常对照组降低(P<0.01);正常对照组左室侧壁的S-str和D-str较左室前壁高(P<0.01)。结论应用二维应变显像技术测定心肌的运动速度和形变有望成为评价心肌疾病的非创性新手段,并可能有助于对心脏疾病过程中病理生理过程变化的理解。
Objective To evaluate and compare myocardium velocity and strain property during ischemia using 2-D strain.Method Eighteen ischemia cases of with less eject fraction(EF) [EF50%,(36%±11)%] and 20 normal controls [EF≥50%,(63±17)%] were enrolled in this study.The systolic velocity(S-Vel),diastolic velocity(D-Vel),systolic strain(S-str) and diastolic strain(D-strain) in all left ventricular walls were respectively measured and compared using 2-D strain.Results The S-Vel,D-Vel,S-str and D-str of all walls in patients group were significantly lower than in control group(P〈0.001);S-str and D-str of lateral wall in normal controls were higher than anterior wall(P〈0.01).Conclusion 2-D strain in mearsuring myocardium velocity and strain maybe a promising and non-invasive method in myocardium diseases evaluation.And may further help us for some physiopathological understanding of heart diseases.
