人类白细胞分化抗原151和基质金属蛋白酶-7在乳腺癌组织芯片中的表达及意义 被引量:2

Expressions of CD151 and MMP-7 in breast cancer tissue microarray and their significance

摘要 目的研究穿膜蛋白人类白细胞分化抗原151(CD151)和基质金属蛋白酶-7(MMP-7)在乳腺癌中的蛋白表达及其与乳腺癌发生、发展、转移和预后的关系。方法构建乳腺癌组织芯片,采用免疫组织化学SP法检测CD151和MMP-7的蛋白表达,探讨二者与临床病理资料之间的关系,并分析二者之间的相关性。定性资料间的比较采用χ2检验、Mann-Whitney检验、Kruskal-Wallis检验和Kappa检验。结果 CD151和MMP-7在正常乳腺组织中表达的阳性率分别为0.0%(0/39)和10.3%(4/39),在原位癌组织中表达的阳性率分别为33.3%(10/30)和40.0%(12/30),在浸润性乳腺癌组织中的表达阳性率分别为59.0%(59/100)和70.0%(70/100),3组间两种蛋白表达的阳性率差异有统计学意义(χ2=41.276,P=0.000;χ2=41.801,P=0.000)。两种蛋白的阳性表达率与乳腺癌患者的年龄及组织学类型无关联(P>0.050),但与淋巴结转移、肿瘤分化程度、TNM分期有密切关系(P<0.050)。此外CD151的表达还与肿瘤大小、雌激素(ER)和孕激素(PR)表达有联系(P<0.050)。乳腺癌中CD151和MMP-7两种蛋白的表达强度具有相关性(Kappa值为0.418,P<0.010)。CD151和MMP-7双阳性患者复发转移率高于对照组患者(χ2=6.159,P=0.013)。结论 CD151和MMP-7在乳腺癌组织中异常高表达并与乳腺癌发生、发展及浸润转移有密切关系。联合检测可作为判断乳腺癌生物学行为和预后的重要指标,其中CD151对ER(-)或PR(-)乳腺癌的判断更具有价值。 Objective To evaluate the expressions of CD151 and MMP-7 (matrix metalloproteinase- 7) in human breast cancer and their relations with the occurrence development, and metastasis and prognosis of breast cancer. Methods By using immunohistochemical SP method, the expressions of CD151 and MMP-7 proteins in breast cancer tissue microarray (TMA) were tested. Their correlations with clinicopathological features were analyzed. The relationship of the two proteins was also analyzed. Comparison between qualitative data was performed using Chi-square test, Mann-Whitney test, Kruskal-Wallis test and Kappa test. Results The positive rates of CD151 and MMP-7 expressions were 0.0%(0/39)and 10.3% (4/39)in normal breast tissue, 33.3% (10/30)and 40.0% (12/30)in tissues of carcinoma in situ, and 59. 0% (59/100)and 70. 0% (70/100)in invasive breast cancer tissues, respectively. The differences of the two kinds of protein expression rates among the three groups were statistically significant (X^2 =41. 276 , P=0. 000;X^2 =41. 801, P=0. 000). The expressions of the two proteins in breast cancer tissues weren't associated with patient's age and histological type (P〉0. 050). But they significantly related with the differentiation degree of cancer tissue, lymph node metastasis and TNM staging (P〈0. 050). Furthermore, CD151 expression significantly related with tumor size and ER/PR expression(P〈0. 050). There was a positive correlation between CD151 and MMP-7 expressions ( Kappa=0. 418,P〈0. 010). CD151 and MMP-7 dual-positive patients had higher rates of recurrence and metastasis than the control group (X^2 = 6. 159, P=0. 013). Conclusions The expressions of CD151 and MMP-7 were over expressed in breast carcinoma, which suggests that the two proteins are related to carcinogenesis, development, invasion and metastasis. Joint detection of the two makers can be used to determine the biological behavior and prognosis of breast cancer. In particular, the detection of CD151 would be more valuable for patients with ER (-) / PR (-) breast cancer.
