摘要
目的了解我院金黄色葡萄球菌的临床分布及耐药性特征,为合理使用抗生素提供依据。方法采用Micro Scan Walk Away40和Pc20药敏板进行菌株鉴定及药敏试验,以sPSs软件对数据进行分析处理。结果金黄色葡萄球菌分离率高的科室是ICU、呼吸内科和脑外科,标本主要来自痰、分泌物和血。168株金黄色葡萄球菌中甲氧西林耐药金黄色葡萄球菌菌株(MRSA)占31.0%,ICU分离的MRSA占52.6%。金黄色葡萄球菌感染的患者中以21~40岁人群为主,占41.0%;60岁以上的人群24.5%,各年龄段男性感染率均较女性高,ICU感染者40岁以下的患者占47.4%,50岁以下的患者占68.4%。ICU患者分离的38株菌痰标本占78.9%,血标本15.8%。万古霉素和利奈唑胺的敏感率为100%,青霉素耐药率为94.6%,红霉素耐药率56.0%、克林霉素46.4%和四环素47.6%。结论ICU、呼吸内科和脑外科是金黄色葡萄球菌感染前三位的科室,主要患者人群为中青年男性,肺部感染为主,其次是血流感染,金黄色葡萄球菌呈高度的耐药性,万古霉素和利奈唑胺是金黄色葡萄球菌体外药敏试验敏感性最高的抗菌药物。
Objective To investigate the clinical distributing and drug - resistant characteristics of staphylococcus aureus. Methods All strains were identified by MicroScan WalkAway40 and performed antimicrobial susceptibility testing by PC20. The SPSS software used to analyze the data. Results Staphylococcus aureus isolated from different samples and in all de- partment in the hospital, mainly collected from ICU, Respiratory Department and Department of Neurosurgery. Sputum, secre- tion and blood were the main source of samples. Methicillin -resistant Staphylococcas aureus were 31.0% of 168 isolates, 52. 6% MRSA of 38 strains in ICU. The age state of patients infected Staphylococcas aureus were mainly 21 -40 years, the pa- tients above 60 years were 24. 5%, 47.4% patients were under 40 years, 68.4% patients were under 50 years in ICU. The Staphylococcas aureus isolates demonstrated antimicrobial resistance to penicillin, erythromycin, clindamycin and tetracycline, 94. 6%, 56.0% , 46.4% and 47.6% respectively. Of the staphylococcas aureus isolates 100% were suscepitible to vancomycin and linezolid. Conclusion Staphylococcas aureus isolates were dominated in ICU, Respiratory Department and Department of Neurosurgery. Staphylococcas aureus infected were mainly male middle -youth age group, pneumoniae and blood stream infection were clinical signs. Staphylococci aureus had the high resistant rates to antimicrobial agents in clinical, and vancomycin and lin- ezolid were still the susceptible antibiotics to all strains containing MRSA, MRSA showed high -level resistance to antimicrobial agents should be pain more attention in the hospital.
