摘要
目的比较开腹全子宫切除术（TAH）、阴式全子宫切除术（TVH）及经腹腔镜全子宫切除术（LTH）对性功能、精神心理的影响。方法对TAH100例、TVH100例及LTH40例,年龄在45~54岁患者术后1年随访,对性功能及精神心理问卷调查的结果进行分析比较,以同样标准选择子宫肌瘤核除术100例为对照组（核瘤组）。结果在性功能方面,与核瘤照组相比,〈50岁组3种途径全子宫切除术对性功能无明显影响（P〉0.05）;而≥50岁组,女性性功能量表FSFI总分,核瘤组〉TVH组〉LTH组〉TAH组,TAH组与核瘤组及TVH组比较,均有显著性差异（P〈0.05）。在精神心理方面,与（核瘤组）相比,〈50岁组3种途径全子宫切除术对精神心理无明显影响（P〉0.05）;而≥50岁组,中国健康问卷（CHQ-12）评分,核瘤组〈TVH组〈LTH组〈TAH组,TAH组与核瘤组及TVH组比较,均有显著性差异（P〈0.05）。结论因子宫肌瘤等良性疾病行全子宫切除术,对年龄小于50岁患者的性功能及精神心理无显著影响,3种全子宫切除术术式之间比较亦无显著性差异;而年龄在50岁以上的患者行开腹全子宫切除术,对精神心理及性功能存在较大的负面影响。
Objective To compare the effects of transabdominal hysterectomy（TAH）,transvaginal hysterectomy（TVH）,and laparoscopic total hysterectomy（LTH）on sexual function and psychological health. Methods A total of 100 patients undergoing TAH,100 patients undergoing TVH,and 40 patients undergoing LTH were followed up for 1 year. All the patients were aged between 45 and 54 years. The results of the questionnaire on sexual function and psychological health were analyzed and compared with those from 100 control subjects undergoing hysteromyomectomy. Results In pateints under 50 years of age,no significant difference in the sexual function and psychological health was found between control group and any one of TAH,TVH,and LTH goups. In patients aged 50 years and older,FSFI score decreased from control group to TVH group to LTH group to TAH group;CHQ-12 score increased from control group to TVH group to LTH group to TAH group;and there was significant difference in the FSFI and CHQ-12 scores both between TAH and control groups and between TAH and TVH groups （P 0.05）. Conclusion There is no obvious effect of these 3 surgical approches to hysterectomy on the sexual function and psychological health in patients under 50 years of age,and there is no significant difference in the sexual function and psychological health among these 3 surgcial approaches. In patients aged 50 years and older,TAH has a negative impact on the psychological health and sexual function.
