新生儿先天性巨细胞病毒感染血β_2微球蛋白检测意义的探讨 被引量:1

Evaluation of the role of serum β_2-microglobulin in neonatal congenital cytomegalovirus infection

摘要 目的探讨新生儿先天性巨细胞病毒(CMV)感染患儿血清β2微球蛋白(β2-MG)检测的意义。方法采用微粒子免疫定量分析法对确诊的新生儿先天性CMV感染患儿(观察组)及同期住院的其他新生儿(对照组)测定血清β2-MG,并进行比较,观察组与对照组病例数比例为1∶2;建立血清β2-MG诊断CMV感染的工作曲线,确定ROC曲线下的面积,同时分析不同截断值的敏感度和特异度。结果观察组共21例,血清β2-MG值为(3112±910)μg/L,对照组共42例,为(2470±624)μg/L,差异有统计学意义(P<0.01);取观察组血清β2-MG3000μg/L为截断值,其敏感度为52.38%,特异度为82.71%。结论血清β2-MG可作为先天性CMV感染诊断的辅助指标,便捷实用,适于基层医院临床开展。 Objective To evaluate the role of serum β2-microglobulin (β2-MG) in neonatal congenital cytomegalovirus (CMV) infection. Methods β2-MG was tested in 21 neonates confirmed congenital CMV infection (observaiton group) and 42 neonates without CMV infection (control group) using microparticle enzyme immunoassay. β2-MG levels were compared between the two groups to assess the effect of different cut-off levels on specificity and sensitivity in the diagnosis of neonatal congenital CMV infection. Results Mean (±SD)β2-MG level were significantly higher in the observation group (3112±910 μg/L) than in the control group (2470±624 μg/L) (P〈0.01). There was a sensitivity of 52.38% and specificity of 82.71% with the cut-off value of β2-MG 3000 μg/L in the observation group. Conclusion The measurement of serum β2-MG is one of an useful adjuvant marker in diagnosing neonatal congenital CMV infection,particularly in hospitals under level Ⅲ.
