抗结核药吡嗪酰胺引起继发性痛风的临床观察 被引量:9

摘要 目的观察抗结核治疗方案中含有吡嗪酰胺药物引起血尿酸升高导致的继发性痛风。方法对60例肺结核患者进行多次血尿酸检测;选择对照组30例患者、治疗组30例患者抗结核。治疗前均排除肾脏疾病、血液疾病、免疫风湿等疾病;检查类风湿因子抗链球菌溶血素"O"均无异常。结果30例服用吡嗪酰胺的患者血尿酸在服药后一至两个月均有不同程度的增高,而血尿酸增高后引起痛风发生的患者仅为3例,另30例未服用吡嗪酰胺的患者在同期治疗未发现血尿酸增高现象。结论抗肺结核药物吡嗪酰胺可引起血尿酸增高导致继发性痛风的发生,发生率约为10%,停药后约15~30d症状逐渐好转直到消失,或服用别嘌醇/丙磺舒5~10d症状可完全消失,也可用布洛芬、大亚芬克、阿司匹林等镇痛药对症处理。
