“翻山鞘”技术在下肢深静脉血栓形成取栓 溶栓中的应用 被引量:4

摘要 目的探讨介入取栓溶栓术治疗下肢深静脉血栓形成的方法与疗效,术中运用"翻山鞘"技术的应用。方法所有患者在介入取栓、溶栓前均放置下腔静脉滤器,而后经健侧穿刺道引入翻山鞘,在导丝配合下,反复推进鞘心和外鞘,直至外鞘完全翻入患侧髂静脉,引入Guiding导管反复抽吸取栓,最后引入溶栓导管进行持续溶栓治疗,并配合足量抗凝治疗。结果应用"翻山鞘"技术降低了手术难度,节约了手术时间,降低了手术风险,术后3 d内症状完全缓解,造影证实血管完全通畅,其中4例应用球囊扩张狭窄髂静脉,术后2个月至半年经彩超复查血管完全通畅。结论下肢深静脉血栓形成通过介入治疗能取得了满意的疗效,而翻山鞘的应用降低了手术难度,节约了手术时间,降低了手术风险。 Objective To investigate the involvement of thrombectomy thrombolysis in the treatment of deep venous thrombosis methods and efficacy of surgery with the use of "crosses sheath" technology.Methods All patients in interventional thrombectomy,thrombolysis were placed before the inferior vena cava filter,and then crosses through the contralateral sheath puncture Road,to introduce,in cooperation with the guide wire repeatedly to promote heart and the outer sheath sheath until the outer sheath completely plunged into ipsilateral iliac vein,the introduction of Guiding Catheter had repeatedly drawn draw bolt and finally the introduction of thrombolysis catheter for continuous thrombolysis and anticoagulation therapy with sufficient quantity.Results All had their symptoms relieved completely within 3 days after operation,angiography confirmed vascular patency completely,including four cases of application of balloon-expandable iliac vein stenosis,vessels were unobstructed by the CDFI two to six months after operation.Conclusion The sheath crosses the application can reduce the difficulty of the surgery,savethe operation time and reduces surgical risk.
