摘要
目的探讨术中放疗病人围术期麻醉管理特点和放射防护经验。方法回顾性总结54例使用移动式电子束术中放疗系统(MOBETRON放射系统)进行肿瘤术中放疗的病例,对围术期的麻醉管理和放射防护经验进行分析。结果术中放疗需要多学科良好协作,其麻醉管理除遵从全身麻醉的常规原则之外,还需要关注麻醉深度控制、病人体位及设备管理、放射防护、病人的放疗增敏、放疗并发症的预防等。结论围术期的充分准备,合理的麻醉管理和有效的工作流程,完善的监测,有助于维持病人呼吸循环稳定和内环境平衡,保证术中放疗的顺利实施。
Objective To investigate the experience about Anesthesia management and radiation protection for intraoperative radiotherapy（IORT）. Methods The clinical material of 54 cases who undertake IORT by MOBETRON were analyzed retrospectively. Result lntraoperative radiotherapy （IORT） is a relatively new modality for the treatment of carcinoma. This modality necessitates a muhidisciplinary approach, anesthesiologist, radiotherapist, pathologist, and other members of the surgical support team. In addition to appropriate IORT and surgical techniques, the role of the anesthesiologist is crucial in determining patient outcome, the preoperative preparation, the control of anesthesia depth, body position, equip management, radiation protection, radiotherapy sensitization, complication prevention et al should also be considered. Conclusion A good preoperative preparation, reasonable Anesthesia management, effective program and consummate monitoring should serve to maintain steady state and make the IORT procedures feasible and safe.
