摘要
目的探讨胎儿肺动脉闭锁-室间隔完整的超声诊断图像特点,并为临床产科处理提供依据。方法回顾性分析6例经解剖或生后超声心动图证实为肺动脉闭锁-室间隔完整胎儿的超声资料,总结其声像特点。结果6例肺动脉闭锁-室间隔完整的病例,四腔心显示右室腔缩小2例、大小正常1例、增大3例,右房均增大(含巨大右房3例),三尖瓣中重度反流4例,狭窄、闭锁各1例;4例肺动脉内径细小,2例肺动脉主干内径正常但肺动脉瓣无过瓣血流显示,4例可见动脉导管逆向血流显像;6例左房左室及主动脉发育正常或稍大。结论超声诊断胎儿肺动脉闭锁-室间隔完整,四腔心切面有很好的提示作用,出现三尖瓣大量反流及巨大右房,应考虑此病的可能;动脉导管反流起间接的提示作用;直接的证据是肺动脉瓣没有过瓣血流显像。
Objective : To explore the uhrasonographic characteristics of fetal pulmonary atresia with intact ventricnlar septum ( PA - IVS), and to supply with the remedy basis for obstetrics. Methods : Six neonates with pulmonary atresia were enrolled in the study, they had intact ventricular septum. PA - IVS was confirmed by autopsy or echocardiography during neonate. To analyze retrospectively the prenatal material that ultrasound suggested and to sum up the characteristics of acoustic image. Results: Six cases with PA - IVS were diagnosed on prenatal uhrasonography, On four- chamber view there were two cases with narrow right ventricle, one case with normal right ventricle and three cases with broad right ventricle, Meanwhile they all had enlarged right atrium (including three cases with huge right atrium) . In six cases, there were four cases with middle - heavy tricuspid valve reversible flow, one case with restricted tricuspid valve and one case with atresic tricuspid valve; Four cases were small pulmonary artery, while two cases were normal pulmonary artery trunk but having no blood flow through pulmonary valve, four cases existed reversible blood flow through the ductus arteriosus. All six cases had normal or bigger left atrium, left ventricle and aorta. Conclusions: Four - chamber view played an important role in diagnosing fetal PA - IVS. When there was a great deal of reversible blood flow through tricuspid valve or vast right atrium, PA - IVS was possible. Reversible blood flow through the ductus arteriosus had an indirect effect on suggesting PA - IVS. However direct evidence diagnosing PA - IVS still was the sign that there was no blood flow through pulmonary valve.
