摘要
目的探讨腹腔镜辅助阴式子宫切除术(LAVH)的并发症及其相关因素。方法回顾性分析357例LAVH的临床资料以及出现严重并发症的相关因素。结果357例LAVH中出现严重并发症7例,并发症的发生率为2%。并发症包括膀胱损伤1例,输尿管阴道瘘1例,肠道损伤2例,内出血2例,盆腔脓肿1例。盆腔粘连、子宫过大、子宫内膜异位症增加手术难度,并发症的发生率较高。结论LAVH并发症与手术难度和术者技巧有关,加强医师手术技巧培训及正确的术前评估有望减少LAVH并发症的发生。
Objective To explore the complications and their relevant factors of laparoscopic assisted vaginal hysterectomy（LAVH）.Methods Clinical data of was retrospectively reviewed and analyed in 357gynecological patients with LAVH and the factors resulting in serious complications from January 2006 to December 2008.Results Among the 357 LAVH cases,seven cases showed serious complications,the rate was 2%.The complications included one bladder injury（one case）,ureterovaginal fistula（one case）,intestinal injury（two cases）,internal hemorrhage（two case） celiac abscess.The general incidence of complications became higher since the operations got more difficult with factors of pelvic adhesion,uterine oversize and endometriosis.Conclusion The complications of LAVH have much to do with the operation difficulty and the skills of the operator.Reinforcement on the training of the operators＇ operation skills and correct evaluation prior to the operation are likely to reduce the incidence of LAVH complications.
