摘要
目的追踪穿透性角膜移植术患者泪液中IL-6、IL-10mRNA动态表达规律，探讨炎症因子在角膜移植创伤愈合中的作用机制。方法2007年7月至2007年12月接受穿透性角膜移植手术的患者20例为研究对象，吸取术前1d、术后1周及3周的泪液，利用实时荧光定量PCR技术检测IL-6、IL-10mRNA表达水平，20名健康人作为对照。结果手术组病例患者术前1d、术后1周及3周泪液中IL-6的CT值分别为18．471±2．698、18．723±2．640、20．706±2．445，对照组泪液中IL-6的CT值为22．801±2．834。IL-10在相应的CT值检测点分别为15．178±1．640、16．042±2．516、16．690±2．296，对照组IL-10的CT值为21．251±3．111。经统计学t检验分析证实手术组患者术眼泪液中IL-6和IL-10的CT值含量在各个检测时间点均低于对照组（P均〈0．01），术后3周患者术眼泪液中IL-6及IL-10的CT值明显高于术后早期检测值（P均〈0．01）。说明PKP术后患者泪液中炎症因子的表达强度呈下降趋势。结论穿透性角膜移植手术患者泪液中炎症反应高于正常人，IL-6和IL-10可作为评价术后炎症反应强烈程度的检测指标，指导术后抗炎药物的规范使用。
Objective To explore the sequencial changes of tear IL-6 mRNA and IL-10 mRNA in patients of penetrating keratoplasty （PKP） surgery, and to understand the the inflammatory response before and after PKP surgery and wound healing mechanism. Methods Selected from July 2007 to December 2007 PKP surgery patients with 20 healthy subjects as a control group. Tear was collected ld before surgery, and lw, 3w after surgery, fluorescence quantitative PCR was used to detect IL-6 mRNA and IL-10 mRNA in tear. Results The CT value of IL-6 at pre-operation, 1 w post-operation and 3w post-operation was 18.471± 2.698, 18.723± 2.640 and 20.706＋ 2.445, respectively, and that of the control was 22.801±2.834. The CT value of IL-10 at the same time was 15.178± 1.640, 16.042± 2.516 and 16.690± 2.296, respectively, that of control was 21.251 ± 3.111. It was indicated that the content of IL-6 and IL-10 in tear in PKP surgery patients was higher than that in control （P〈0.01）, the content of IL-6 and IL-10 in tear trended to descend after surgery, compared with the level of early post-operation, the level of IL-6 and IL-10 in tear at 3w post-operation was decreased significantly （P^0.01）. Conclusions The level of inflammatory factors in tear in patients of PKP surgery was higher than that of health human. IL-6 and IL-10 could be acted as markers of inflammatory reaction afer PKP surgery, and they could guide the rational usage of anti-inflammatory drugs after the surgery.
