摘要
目的:通过对大肠腺瘤早期癌变核形态的量化分析,探讨其变化规律及应用于临床诊断的可能性。方法:应用图像分析的方法,对139例大肠病变标本进行核形态定量学研究。结果:腺瘤不典型增生至早期癌变,其细胞核形态的各项参数值均随恶性程度的增加而增大,组间差别有显著性(P<0.01)。通过参数逐类判别建立的函数式,其四代矩阵分析与原诊断的符合率为71%.结论:核形态图像量化分析对大肠腺瘤早期癌变具一定的鉴别能力,建立的函数判别式显示出一定的判别功能。
Objective:To search for a more reliable diagnostic discriminant in early colorectal adenomatou malignancy.Methods:The specimens of 139 cases with biopsy and surgery were measured by neuclear morphological image analysis.Results:The discriminant analysis showed that statistically highly significant differences existed in nuclei measured in adnomas with dysblasia and in adnomatus malignacy(P<0.01).The original data were analysed by a computer and function of diagnostic discriminant was set up with SPLM statistic sofeware.The study of regress showed the sensitivity of adnoma with mild dysplasia,adnoma with moderate dysplasia,adnoma with severe dysplasia,and adnomatus early malignancy were respectively 82%,65%,67% and 67%.Conclusion:The functional formula based on the above parameters is expressive of automatic discriminatory ability.
