摘要
目的回顾比较部分泌尿外科疾病应用后腹腔镜技术与开放手术的治疗,总结临床经验,提高后腹腔镜技术水平,逐步扩大后腹腔镜技术在泌尿外科疾病的应用范围。方法采用后腹腔镜技术治疗泌尿系疾病137例(A组),开放手术130例(B组),两组分别就手术时间、出血量、术后排气时间、术后住院日等进行比较。结果两组疗效无显著差异(P>0.05),后腹腔镜组术中及术后出血量、平均住院时间均明显少于开放组(P<0.05),部分手术时间略长于开放组,有些手术时间短于开放组。结论后腹腔镜技术应用于泌尿外科疾病,途径直接,不会对腹腔镜内脏器造成干扰和污染,且损伤小、痛苦少、康复快、住院时间短。随着不断训练、不断熟练和技术水平的提高加之相关操作器械的更新,部分手术时间的缩短不容置疑,将在泌尿外科领域得到更广泛的应用。
Objective To review and analyze the application of retroperitoneal laparoscopy and open surgery in some urologic diseases over the past two years,sum up clinical experience,and improve the skills of retroperitoneal laparoscopic surgery to gradually expand the application of laparoscopy in urology diseases.Methods Retroperitoneal laparoscopic surgery was carried out in 137 cases of urinary tract diseases （group A）.Open surgery was carried out in 130 cases （group B）.The two groups were compared in operative time,blood loss,postoperative discharge time and postoperative hospital stay respectively.Results Efficacy of two different surgical Methods was the same.Group A was significantly less than group B in intraoperative bleeding time,postoperative bleeding time and average length of stay.Operative time in most cases of group A was slightly longer than that in group B,while it in some cases of group A was shorter than that in group B.Conclusion Retroperitoneal laparoscopic technology is suggested in the treatment of urological diseases for its direct way,non interference and pollution to laparoscopic internal organs,less damage,less pain,rapid recovery and short hospital stay.With ongoing training and constantly updating technical skills and equipments,retroperitoneal laparoscopic technology will shorten the operative time,and will be more widely used in the field of urology.
