摘要
目的应用二维应变超声心动图(2DSE)技术检测心梗大鼠左室壁的径向、周向应变及应变率,以组织学上心肌梗死面积为标准,探讨2DSE技术对评价梗死心肌的价值。方法开胸制备17只大鼠急性心梗模型,于术前24 h、术后4周分别行超声心动图检查,应用2DSE技术分析左室短轴乳头肌水平各节段收缩期峰值径向、周向应变及应变率,心肌梗死后4周心脏标本行组织学染色后计算梗死心肌面积。结果(1)与术前相比,术后4周各节段径向、周向应变及应变率均显著降低,以梗死区降低最为显著(P<0.05);(2)2DSE的径向、周向应变及应变率指标与梗死面积均呈显著负相关,以径向应变<10%判断节段心肌梗死面积>30%的灵敏度和特异度分别为0.84,0.98。结论2DSE技术可用于评价心肌梗死大鼠局部心肌功能,对判断节段心肌梗死面积>30%有较高的准确性。
Objective This experimental study was aimed to investigate the correlations between regional myocardial systolic strain and infarct area in a rat myocardial infarction model by 2-dimensional strain echocardiography (2DSE). Methods An animal model of myocardial infarction was established by opening chest and ligating the proximal left anterior descending coronary artery (LAD) in 17 SD rats. Regional myocardial function was detected by 2DSE at baseline and 4-weeks post-infarction, including end- systolic radial strain and strain rate (SR and SrR) and end-systolic circumferential strain and strain rate (SC and SrC) of each of 6 segments. The size of myocardial infarction was calculated by Masson staining in histology. Results (1) Comparing with parameters measured at baseline, there were significant decreases in SR, SrR, SC and SrC of each of 6 segments at 4-weeks post-infarction, especially in the infarct area(P〈0.05) ; (2) Significant negative correlations were found between 2DSE parameters and size of myocardial infarction(P〈0.05), and SR less than 10% has 84% sensitivity and 98% specificity for detecting segments with infarct area greater than 30%. Gonclusions 2DSE is able to assess regional myocardial function in a rat myocardial infarction model and has high accuracy in detecting segments with infarct size greater than 30 %.
