摘要
目的:探讨环氧化酶-2(COX-2)在舌鳞癌以及癌前病变中的表达意义。方法:免疫组织化学染色法检测舌鳞癌(47例),舌上皮性异常增生(12例)及正常舌组织(10例)COX-2的表达。分析COX-2表达与病理诊断、舌鳞癌术前化疗、分化程度、TNM分期和淋巴结转移的关系。结果:COX-2在正常舌组织中不表达,舌鳞癌中表达总阳性率为63.8%,舌上皮性异常增生中表达阳性率为31.3%,舌癌组与舌正常组织及舌上皮异常增生组的差异有统计学意义,P=0.000。其中在舌癌发生转移组和未发生转移组中COX-2表达的差异无统计学意义,P=0.750。舌鳞癌中是否接受术前化疗、临床分期和病理分级各组中的COX-2表达差异均无统计学意义,P>0.05。结论:COX-2在舌鳞癌及癌前病变中存在不同程度的高表达,对舌鳞癌的发生和发展起着重要作用,舌鳞癌各分组中COX-2的表达差异无统计学意义。提示COX-2可能是一个舌鳞癌靶向治疗的标志。
OBJECTIVE： To investigate the expression and sig nifieanee of cyclooxygenase 2 （COX-2） in tongue squamous cell earci noma （TSCC）and tongue precancerous lesion. METHODS： The immu nohistochemistry was taken to detecte the different expressions of COX -2 in 3 groups：TSCC （47 cases）, tongue precancerous lesion （12 cases） and normal tongue tissue（10 cases）. The relationships between the expressions of COX- 2 and the pathological characteristics, preop erative chemotherapy, TNM staging, cell differentiation, and lymph node metastasis were analysed respectively. RESULTS： COX-2 was not expressed obviously in normal tongue tissues, while expressing a positive rate as 63.8% in TSCC and 31. 3% in tongue epithelial dysplasia. Compared to normal tongue tissues and tongue epithelial dys plasia, the expression of COX -2 was significantly stronger in TSCC （P=0.000）. There was no significant difference of COX-2 expressions in metastasis group and non metastasis group（P= 0. 750）. The expression of COX- 2 in TSCC had no relation with preoperative chemotherapy, clinical states, lymph node metastasis and the degree of cell differentiation（P〉0.05）. CONCLUSIONS： COX-2 has different ex pression levels between TSCC and tongue precancerous lesion, which plays an important role in pathogenesis and development squamous cell carcinoma of tongue. The slight diffrence of expressions of COX -2 in different groups of carcinoma tissues indicates that COX-2 may be a sign of targeted therapy.
