摘要
目的观察光动力疗法(PDT)治疗孤立型脉络膜血管瘤的疗效。方法对眼底荧光素血管造影(FFA)及吲哚青绿血管造影(ICGA),确诊为孤立型脉络膜血管瘤的9例患者的9只眼,经进行PDT治疗。选用689nm红外激光,激光斑直径根据瘤体大小选择,当肿瘤直径大于激光斑时可选择,在肿物表面联结几个光斑将其覆盖。治疗功率选用50J,照射时间为每个光斑125s。治疗后1周复诊,以后每隔3个月复诊一次,每次复诊均常规检查视力、散瞳检查眼底、行彩色眼底照像、FFA及ICGA检查。如发现病灶仍有活动性渗漏,重复相应的治疗。随访时间最短6个月,最长36个月,平均13.6个月。结果治疗后视力与治疗前视力、肿瘤位置以及合并浆液性视网膜脱离的程度、时间和黄斑有无囊样变性有关。除一例患者黄斑囊样水肿明显、术后视力未提高外,其余患者均提高二行以上。间接检眼镜检测,肿瘤区域不透红光,浆液性视网膜脱离消失,FFA检测早期病变处为不显荧光的暗区,未见有血管形态的强荧光,个别患眼于其边缘处有窗样荧光透见,荧光素渗漏减少或消失,晚期无明显的强荧光持续。ICGA检测,早期及晚期均为不显荧光暗区。9例患者在PDT治疗过程中及治疗后未发生任何全身和局部不良反应。结论PDT是有效治疗的脉络膜血管瘤方法,尤其适用于黄斑中心及视盘旁的肿瘤。
Objective To evaluate the therapeutic effects of photodynamic therapy (PDT) on symp- tomatic circumscribed choroidal haemangioma. Methods Clinical data of 9 cases (9 eyes) who had under- gone PDT because of symptomatic circumscribed choroidal haemangioma, which had been diagnosed by ex- amination of ocular fundus, fluorescein angiography (FFA), and indocyanine green angiography (ICGA). Diode laser 689nm was used with an intensity of600mW/cm2 for 125 seconds. Single or multiple partially over- lapping spots were applied based on the tumor basal dimensions. Periodic follow up with ophthalmoscope, vi- sual acuity and angiographic studies were performed. Results All 9 patients showed evidences of regression with flatting of tumor, resolution of subretina fluid, and reduction of choroidal vasculature on angiograms. The visual acuity either improved. There were no photosensitive reactions or systematic adverse events occurred. Conclusions Verteporfm PDT is an effective treatment for the management of symptomatic circumscribed choroidal haemangioma.
