摘要
目的总结腓肠内侧动脉穿支游离皮瓣的临床应用结果。方法用这种方法修复6例手和足部软组织缺损,其中手掌侧缺损1例,手背3例,足背2例,供区均选用同侧小腿。结果1例术后发生表浅感染,经交换敷料逐渐愈合,皮瓣全部成活,术后经过顺利,取得了较满意的效果。没有发现明显的供区功能障碍。结论该皮瓣以腓肠内侧动脉的肌皮穿支为血供,具有血供丰富,血管解剖恒定,血管蒂长,以及皮瓣较薄的优点,适宜修复手和足背部软组织缺损。
Objective To summarize clinical application of the repair soft tissue defect of the hand and foot with the medial sural artery perforator free flap by microvascular anastomoses.Methods Six patients with soft tissue defect of the hand and foot underwent reconstruction with this method.Of them,1 case recipient site was located on the palm aspect of the hand,3 cases were located on the dorsal aspect of the hand,2 cases was located on the dorsal aspect of the foot.The donor leg was ipsilateral in all cases,Results One case sustained superficial infection postoperative and the gradual wound healed by daily wound dressings.All the flaps had survived completely without major complication and with satisfactory clinical results,There was no remarkable donor site morbidity.Conclusion The flap is nourished by the musculocutaneous perforater of the medial sural artery.The flaps seem to has highly vascularize,a constant vascular anatomy and a long vascular pedicle,The flap is thin and suitable for repair of soft tissue defect of the dorsum of the foot and hand.
