摘要
目的探讨过屈过伸位功能摄片在腰椎疾病X线诊断中的应用价值。方法对150例腰腿痛患者进行过屈过伸腰椎侧位片检查,观察腰椎椎体向前及或向后移位、滑脱情况和棘突分离或相互靠近情况。结果经Meyerding法测量,过伸位显示存在椎体滑脱者93例,过屈位显示椎体滑脱者29例;过屈位棘突局部不能分离者35例,过伸位棘突局部不能靠近者17例。结论过屈过伸位检查常能够发现常规腰椎正侧位及双斜位所不能显示的腰椎失稳和功能改变。有时还能发现常规片中不能显示或显示不清楚的解剖结构性改变。对X线观察腰椎活动,判断腰椎失稳以及估计病人治疗后功能恢复的程度有一定作用。
Objective To evaluate the value of flexion-extension radiography in the diagnosis of lumbar spondylosis. Methods Flexion-extension lumbar spine radiography was.performed on 150 patients with lumbar spondylosis. The gliding of each lumbar vertebral body forward or backward, and the separation or close approximation of the spinous processes were also evaluated. Results Under the Meyerding measuring method, 93 patients showed the separating of the vertebral bodies on the extension radiographs, and 29 ones on the flexion radiographs; 35 patients failed to show the separation of the regional spinous processes on the flexion films, while 17 ones could not reveal the close approximation of the spinous processes on the extension films. Conclusion The flexion-extension radiography may reveal not only the lumbar instability and the limiation of lumbar movement but also the anatomical structural changes which could not be seen on the conventional lumbar spine radiographs. It plays an important role not only in the observation of the lumbar movement and the diagnosis of lumbar instability, but also in the evaluation of the functional improvement after the therapeutic measures.
