摘要
宫内节育器(IUD)避孕是计划生育的有效常用方法。IUD取出是保障IUD放置的有效措施,但IUD取出困难(简称取器困难)及其并发症是影响IUD避孕的主要原因。取器困难的主要类型和原因是围绝经期致子宫萎缩、生殖器炎症致宫腔粘连、IUD嵌顿或迷失等。主要的处理方法有正确判断子宫位置大小、宫颈管软化和扩张、弯钳协助取器、宫腔镜或B超引导下取器、围绝经期辅助用药取器等,方法不一,各有利弊。本文就取器困难的原因进行分析,并一一阐述其临床处理方法。
Intrauterine device （IUD） is an effective and frequently used method in birth control. The IUD removal is the effective measure in ensuring the IUD set. The difficult intrauterine device removal and its complication are the primary causes that affect contraception. This paper analyzed that the main causes of difficult intrauterine device removal included metratrophia in perimenopausal period, metrosynizesis because of edeitis, IUD incarceration or disorientation. The main treatments included correct judgment of uterus position and size, softening and dilating cervical canals, and removing IUD assisted with curved forceps, hysteroscopy, B-mode ultrasonic diagnostic equipment, or with drugs in perimenopausal period.
