吲哒帕胺治疗22例高血压病的动态血压监测

摘要 目的:评价吲哒帕胺治疗高血压的疗效和安全性。方法:予22例轻中度的Ⅱ期高血压病患者服吲哒帕胺(2.5~5mg/d)8周,利用24h动态血压监测和常规血压观察其疗效,并监测患者血糖、血K+等生化指标。结果:常规血压和动态血压显效率均为90.9%(22例),总有效率均为100%(22例)。22例治疗前后动态血压监测示:吲哒帕胺能显著地降低24h、白昼和夜间的平均血压和降低夜间/白昼血压均值比值(即提高夜间血压下降率)(P均<0.01),同时吲哒帕胺的降压效应谷峰比值:SBP(0.64±0.09),DBP(0.66±0.08),均>0.5。未发现明显的不良反应且对血糖等无影响,但可使部分患者血清K+和Na+于正常范围内的明显下降(P<0.05和P<0.01)。结论:吲哒帕胺是一种安全、有效、能持续地控制24h血压的降压药物,并具有纠正患者异常的昼夜血压节律和具有良好的降压效应谷峰比值作用。 Objective: To evaluate the antihypertensive efficacy and safety of indapamide. Methods: Twenty two patients with stage Ⅱ of mild and moderate essential hypertension (EH) were treated with daily 2.5~5 mg indapamide orally for 8 weeks.Dynamic blood pressure mornitoring (ABPM) was taken before and after indapamide treatment. Blood glucose,serum potassium, etc,were tested too. Results: The significantly effective rate of indapamide on hypertension in routine blood pressure measurement and ABPM were similar and both total effective rates were the same 100%. The effect of indapamide by ABPM was obvious on controlling mean blood pressure (daytime,nighttime and 24 hours), it lowered average blood pressure nighttime/daytime ratio. Trough: peak ratio of blood pressure response to indapamide was over 0.5 (SBP 0.64±0.09,DBP 0.66±0.08). Low incidence of adverse effect was observed during indapamide treatment.The result showed that blood glucose,cholesterol, uric acid, urea nitrogen and serum creatinine did not significantly increase,but seru potassium ( P <0.05) and sodium ( P <0.01) did significantly decrease and both were still at normal level. Conclusion :Indapamide is a safe,effective antihypertensive agent,it can controll 24 hour blood pressure, and correct abnormal regularity of daytime and nighttime blood pressure of patients with hypertension,and Trough: peak ratio of blood pressure response to indapamide is better.
