摘要
目的探索类风湿关节炎(RA)患者骨密度(BMD)与部分免疫指标的相关关系,为防治RA继发性骨质疏松症提供参考。方法选择RA住院患者25例,记录患者年龄、病程、体重指数等基本情况,用GE Lunar Prodigy型双能骨密度仪检查腰椎与髋部骨密度,抽血查IgA、IgG、IgM、C3、C4、CRP、RF、ESR等实验室指标,用Epi Data3.1录入数据,用SPSS11.5进行统计分析,用Pearson方法分析观察指标之间的直线相关关系。结果RA患者腰椎和髋部BMD均与年龄呈显著负相关(r=-0.576,P=0.003;r=-0.632,P=0.001);腰椎BMD与RF呈显著负相关(r=-0.571,P=0.033);髋部BMD与体重指数呈正相关(r=0.547,P=0.006),与ESR呈负相关(r=-0.565,P=0.008)。结论RA患者腰椎和髋部BMD有着不同的影响因素;降低RF和ESR、增加BMI有利于增加RA患者腰椎和髋部BMD;RA患者应尽早预防骨量丢失。
Objective To investigate the correlations between bone mineral density （BMD）and some immunity parameters in patients with rheumatoid arthritis （RA）in order to provide the experimental basis for preventing and treating secondary osteoporosis（OP）after RA. Methods In this study,26 patients were enrolled. The patients＇ age, illness course and body mass index （BMI） were recorded. The BMD in lumbar and hip was measured by DXA （ GE Lunar Prodigy）. The levels of IgA, IgG, IgM, C3, CA, CRP, RF, ESR in blood were examined. The data were analyzed with SPSS 11.5 software. The linear correlations of indexes were observed by Pearson method. Results The BMD in lumbar and hip was negatively correlated with patients＇ age （ r = -0. 576, P 〈 0.01 ; r = -0.632, P 〈 0.01 ）. There was a significant negative correlation between BMD in lumbar and RF （ r = -0. 571, P 〈 0.05 ）. The BMD in hip was positively correlated with BMI （ r = 0. 547, P 〈 0.01 ）, however,which in hip was negatively correlated with ESR （ r = -0. 565, P 〈 0. 01 ）. Conclusion The effect factors on the BMD in lumbar and hip of patients with RA are different. The decrease of RF and ESR and increase of BMI are helpful to enhance the levels of BMD in lumbar and hip of patients with RA, so that the patients with RA should prevent the loss of bone mass as early as possible.
