成人门静脉海绵样变性16例诊治分析 被引量：1

摘要 目的:探讨成人门静脉海绵样变性的诊断和治疗方法。方法:回顾性分析16例成人门静脉海绵样变性患者的临床资料。结果:16例均经彩色多普勒超声检查确诊。1例因上消化道大出血急诊行脾切除+贲门周围血管离断术,2例既往已行脾切除加断流术后再出血者再次断流后行限制性肠系膜上静脉-下腔静脉人工血管架桥术,6例行脾切除+贲门周围血管离断联合限制性肠系膜上静脉-下腔静脉人工血管架桥术,4例行脾切除+贲门周围血管离断联合脾静脉-下腔静脉人工血管架桥术,2例脾切除+贲门周围血管离断联合脾-肾人工血管架桥术,1例行改良脾-肺固定术。16例患者均获随访,除1例手术再次复发出血外,余15例患者术后情况均稳定。结论:彩色多普勒超声是成人门静脉海绵样变性首选的诊断方法,脾切除+断流联合分流术是门静脉海绵样变性的首选术式。
