摘要
目的探讨乙型肝炎后肝硬变(PHC)患者外周血淋巴细胞(PBL)IL2活性,IL2R表达与肝功能的关系.方法PHC患者56例,按ChildPugh分级法将其肝功能状态分成A,B,C三级,检测其IL2R的表达,IL2的生成及血清可溶性IL2受体(sIL2R)的水平,并与24例健康成人进行比较.结果PHC组PBL经PHAP诱导的IL2R表达和IL2生成均显著低于对照组(t=2270和4633,P<005和P<001),而血清sIL2R水平则显著高于对照组(t=6219,P<001).PHC组和对照组的IL2活性与IL2R表达率之间呈正相关(PHC组:r=0414,P<001;对照组:r=0804,P<001),与血清sIL2R水平则呈负相关(PHC组:r=-0506,P<001;对照组:r=-0422,P<005).PHC患者随着肝功能恶化(ChildPughA,B,C三级之降低),PBL经PHAP诱导的IL2R表达和IL2生成亦越低,而sIL2R水平则越高.结论PHC患者IL2R系统的功能有障碍与缺陷,而且这种障碍与缺陷与其肝功能状态有密切关系?
AIM To explore the correlation between liver function states and IL 2 activity and IL 2R expression in patients with post hepatitis B cirrhosis (PHC). METHODS IL 2 production and IL 2R expression on peripheral blood lymphocytes (PBL) induced by PHA P, the serum level of soluble IL 2 receptor in 56 patients with PHC and in 24 healthy adults as control were measured by the modified MTT method, flow cytometry assay and ELISA method, respectively. RESULTS〓IL 2R expression and IL 2 production in PHC patients were much lower than those in the control group ( t =2 270 and 4 633, P <0 05 and P <0 01 , respectively), but serum sIL 2R level was significantly higher than that in the control group ( t =6 219, P <0 01 ). A significant linear correlation was found between IL 2 production and IL 2R expression (PHC group: r =0 414 , P <0 01 ; control group: r =0 804 , P <0 01 ), while a significant reverse correlation was found between IL 2 activity and serum sIL 2R level (PHC group: r =-0 506 , P <0 01 ; control group: r =-0 422 , P <0 05 ). With Child Pugh's classification of cirrhosis, we found that the poorer the liver function, the lower the IL 2R expression and IL 2 production, and the higher the sIL 2R level. CONCLUSION The function of IL 2 system in patients with PHC was decreased and closely correlated with their liver function state.
