摘要
目的:评价涎腺内镜对慢性阻塞性颌下腺炎的诊治价值。方法:收集了2006年9月至2009年8月因颌下区反复出现肿胀就诊于我院的患者共48例,其中男性30例,女性18例,年龄12～62岁。所有患者术前常规行下颌横断咬合片检查,26例行颌下腺造影,48例均接受涎腺内镜检查。结果:咬合片显示颌下腺导管阳性结石18例,造影片显示导管内阴性结石15例,主导管及分支导管不同程度的扩张。涎腺内镜显示:颌下腺导管内结石40例(包括术前诊断为阴性结石15例及漏诊7例患者),导管狭窄6例,扭曲2例。所有患者均伴有不同程度的导管炎性改变,如导管内壁片状充血,管壁苍白,粘液栓等。所有患者术后颌下区肿胀症状均明显减轻,未发生并发症。结论:涎腺内镜是诊治慢性颌下腺炎的一种安全、微创、有效的方法。
Objective:To evaluate the clinical value of sialoendoscopy in the treatment of obstructive submandibular gland diseases.Methods:21 patients who complained recurrent swelling in the submandibular gland region were selected for sialoendoscopy therapy from Sept. 2006 to Aug. 2009 at the department of Stomatology,the Centre Hospital of Jinzhou,including 30 males and 18 females.Their ages range from 12 to 62.All the cases received a standard mandibularocclusal film examination before sialoendoscopy,and 26 cases underwent sialograph,48 cases were examined by Sialoendoscopy.Results:Conventional radiographs revealed that 18 cases had submandibular gland duct radiopaque stones,and 15 cases were discovered with radiolucent stones by sialograph.Sialoendoscopy examinations revealed that 40 cases had sialolith,strictures were identified in 6 cases and kiks in 2 cases. All patients were accompanied by varying degrees of duct inflammatory changes, such as erythema on the wall of the duct,white appearance of ductal layer,mucoid plugs ect.The symptoms of submandibular area swelling were significantly reduced after operation in all cases, without any complication.Conclusion:Sialoendoscopy is a kind of promising new technique.It may be a safe,minimal invasive and effective method in the diagnosis and treatment of submandibular gland diseases.
