摘要
目的探讨CT检查对成釉细胞瘤的诊断价值。方法回顾性分析18例经病理证实的成釉细胞瘤的CT表现,观察病变的形态和结构特征。结果18例中,12例位于下颌骨磨牙区及升支部,2例位于下颌体部,4例位于上颌骨。CT表现：18例中,病变呈多房型13例,单房型5例,其中多房型者10例边缘呈分叶状,5例可见不完全骨性分隔。病变以向唇颊侧膨胀为主,边界较清,6例穿破骨皮质,3例在局部形成软组织肿块;病变区密度不均,CT值范围为9.6~30.4 Hu,平均20.2 Hu;10例牙根呈截断样吸收;15例有邻近结构的侵犯。结论CT检查对成釉细胞瘤的诊断及鉴别诊断有重要价值。
Objectives To study the value of CT examination in the diagnosis of ameloblastoma of the jaw.Methods CT features of ameloblastomas,which were proved by pathology,were retrospectively studied,and the configuration and structure of the lesions were observed.Results Among the 18 cases,12 lesions located in molaramus region of mandible,2 lesions located in body of mandible,4 lesions located in maxilla.On CT images,13 lesions were mostly multilocular,the other were unicystic,10 of them appeard as lobulate lesions,5 had incomplete bone sepa.The lesions could destroy adjacent cortex,the bone expansion changes were mostly with lip or chap,all of them showed well-defined margins,6 showed cortical perforation,3 had soft tissue masses;CT value of the lesions ranged from 9.6 Hu to 30.4 Hu（mean 20.2Hu）;Root apex resorption generally demonstrated truncation or serration type in 10 cases.The adjacent vital structures were always invaded by the lesions.Conclusion The application of CT could improve the diagnostic accuracy of ameloblastomas of the jaw.
