结核性腹水和恶性腹水的鉴别 被引量:1

The differential diagnosis between tuberculous and malignant ascites.

摘要 目的探讨肿瘤标志物,PPD实验,结核抗体及影像学检查在结核性腹水和恶性腹水中的意义。方法笔者分析了结核性腹水12例,恶性腹水19例,这些患者均进行了肿瘤标志物PPD实验,血清结核抗体等检查。结果联合CA199,CA125(高于1.5倍)和CEA、AFP时,对肿瘤诊断的阳性率高于单用一种肿瘤标志物。传统常规影像学检查阳性率78.9%。结论联合检测CEA、CA125(超过正常值1.5倍)、AFP、CA199有助于结核性腹水和恶性腹水的鉴别,传统常规及影像学检查仍然是帮助结核性腹水和恶性腹水鉴别的重要手段。 Objective To investigate the value of tumor markers and some imaging Method in tuberculous and malignant aseites. Methods The levels of CA199, CA125, AFP, CEA in blood serum and TB -Ab were measured, and other methods had been used in tuberculous( 12 cases) and malignant( 19 eases) ascites. Results The sensitivity of diagnosis of malignant aseites by combining determining the level of CEA, CA199,AFP and CA125 was higher. The positive rate of imaging examinations is 78.9%. Conclusion Combined detections of CEA,CA199, AFP and CA125 have certain value to differential diagnosis of tuberculous and malignant ascites. But imaging examinations is still very important for diagnosis of tuberculous and malignant ascites.
