摘要
目的对TCT和阴道镜活检病理结果进行对比分析，并进一步探讨二者在宫颈癌前病变诊断中的价值。方法回顾性分析209例阳性宫颈的液基细胞学和对应的阴道镜活检病理检查的结果，进行对比研究。结果209例阳性TCT病例中，ASCUS97例（46．41％）、LSIL56例（26．79％）、HSIL52例（24．88％）、鳞状细胞癌4例（1．92％）。209例经阴道镜下多点活检，病理诊断如下：炎症47例（22．49％）、CINI级70例（33．49％）、CINII级32例（15．31％）、CINIII级51例（24．40％）、浸润癌9例（4．31％）。与病理结果比较，209例TCT报告中，LSIL的诊断符合率58．93％（33／56），HSIL及以上病变的诊断符合率92．86％（52／56），LSIL与HSIL及以上搞变的诊断符合率比较，差异有显著性（x^2=7．58，P〈0．01）。结论TCT作为宫颈病变的初筛手段，能够发现大量的宫颈癌前病变患者，结合阴道镜下活组织检查，大大提高了宫颈癌前病变的检出率。
Objective To making a contrasting analysis of TCT and the pathology result of Colposcope biopsy, and discussing their value further in diagnosis of precancerous lesions of uterine cervix . Methods Analyzing retrospectively TCT of positive cervix of 209 cases and corresponding pathology result of colposeope biopsy. Results Among the 209 positive TCT cases ,97 cases are ASCUS（46.41% ） , 56 cases are LSIL （26.79%）, 52 cases are HSIL（24.88% ） ,and 4 cases are Epidermoid carcinoma（ 1.92% ）. Through multiple punch biopsy under colposcope,pathology diagnosis are as below ： inflammation 47cases （ 22.49% ） , CIN I 70cases （ 33.49% ） , CIN II 32cases （ 15.31% ）, CIN III 52cases （24.88 % ） ,infiltrating carcinoma 9cases （4.32%） . Contrasting with pathology result, among the TCT report of 209 cases, the diagnosis coincidence for LSIL is 58.93% （33/56）, but for HSIL and the above - mentioned diagnosis of pathological changes, it is 92.86% （52/56）, which showing obvious deference. （X^2 = 7.58, P 〈 0.01 ）. Conclusion As a preliminary screening means of pathological process of cervix,TCT can find a lot of sufferers from precancerous lesions of uterine cervix, which greatly improve the relevance ratio, combining colposcope biopsy.
