摘要
目的评价高速涡轮钻法拔除下颌高位倾斜阻生(单纯邻牙阻力)、低位水平阻生(单纯骨阻力)、低位倾斜阻生(牙阻力并发骨阻力)智齿的临床可行性及其优势性。方法对120颗下颌阻生智齿随机采用凿骨劈冠术(常规组)或高速涡轮钻法(实验组)去除骨阻力两种拔除方法,进行临床观察分析,记录患者发生骨裂、断根、干槽症的发病情况。结果实验组发生骨裂的例数比常规组少(P<0.001),实验组断根发生率小于常规组(P<0.001),两组干槽症的发病率差异有统计学意义(P<0.001),实验组发病率明显低于常规组(P<0.001),实验组对手术效果表示满意的例数多于常规组(P<0.001),无恐惧配合治疗的例数实验组多于常规组(P<0.001)。结论高速涡轮钻去阻力法能减少骨裂、断根和干槽症的发病率,能减轻病人的恐惧情绪,病人乐于接受,值得临床推广。
Objective To evaluate the clinical feasibility and superiority of high - speed air turbine handpiece in extraction of impacted mandibular third molars in terms of high slope impacted ( pure adjacent teeth resistance), low horizontally impacted ( pure bone resistance), low slope impacted ( teeth resistance combined with bone resistance). Methods 120 impacted mandibulars third molars were randomly divided into two groups to observe clinical reactions of aches, the opening of mouth, incidence of dry socket : comventional group released bone/ crown resistance by traditional chisel, test group by high - speed air turbine handpieee. Results There were significance differences of the rate of bony crack, incidence of root pruning and dry socket between two groups ( P 〈 0. 001 ). The ease to be satisfied with surgery effect and accept therapy without any fear in testing group is more than these in conventional group ( P 〈 0.001 ). Conclusion High - speed air turbine handpiece can reduce the incidence of bony crack, root pruning and dry rocket effectively and ease patients' fearful emotion to accept therapy.
