摘要
目的探讨良性前列腺增生(BPH)患者前列腺体积参数与尿动力学检查结果的相关性,评价各体积参数对膀胱出口梗阻的诊断价值。方法回顾分析48例BPH患者的临床资料,术前行经直肠前列腺彩色超声及尿动力学检查,测量前列腺总体积(TPV)、前列腺移行区体积(TZV),计算移行区指数(TZI),行尿流率、压力-流率测定,利用直线相关分析以及单因素方差分析探讨前列腺体积参数与尿动力学检查结果的相关性。结果患者平均年龄(77.53±7.65)岁。TPV与TZV的相关系数为0.93,P<0.01;TPV与TZI的相关系数为0.36,P<0.05;TZV与TZI的相关系数为0.65,P<0.01。TPV、TZV、TZI与最大尿流率(Qmax)的相关系数分别为-0.06、-0.35、-0.32(P>0.05,<0.05,<0.05)。TPV、TZV、TZI与梗阻系数(AG)的相关系数为0.25、0.29、0.33(P>0.05,<0.05,<0.05)。40ml≤TZV<60ml时,TZV与AG成正相关(r=0.29,P<0.05)。结论TZV、TZI均影响膀胱出口梗阻(BOO)的程度,TZI对BOO的影响大于TZV,可作为临床评估BPH对排尿影响的重要参数。
Objective To investigate the relationship between volume parameters of prostate and urodynamics, and to explore the diagnostic value of volume parameters of prostate for bladder outlet obstruction (BOO) in patients with benign prostatic hyperplasia (BPH). Methods A total of 48 patients with BPH had been tested by transrectal ultrasound and urodynamies from January 2004 to December 2007. Bivariate correlations and One-Way ANOVA were used to calculate the correlation between volume parameters of prostate and results of urodynamics. Results The relative coefficients between total prostate volume(TPV) and transition zone volume(TZV) was 0.93(P 〈 0.01). The relative coefficients between transition zone index(TZI) and TZV was 0.65(P 〈 0.01). The relative coefficients between TPV and TZI was 0.36 (P 〈0.05). The relative coefficients between TPV,TZV,TZI and Qmax were -0.06,-0.35 and -0.32 (P 〉 0.05, 〈 0.05, 〈 0.05). The relative coefficients between TPV,TZV,TZI and AG number were 0,25,0.29 and 0.33 respectively (P 〉 0.05, 〈 0.05, 〈 0.05). Positive correlation was found between TZV and AG number (r = 0.29,P 〈 0.05) when 40 ml≤TZV〈60ml. Conclusions The volume parameters of prostate such as TZV and TZI, rather than TPV, can predict the BOO and its severity of BPH patients. According to relative coefficlents,the TZI correlates more than TZV. We should pay more attention to TZI when treating BPH.
