摘要
目的研究动脉灌注联合全身化疗治疗晚期软组织肉瘤的近期疗效与毒性反应。方法10例晚期软组织肉瘤患者经右股动脉穿刺,行主要肿瘤供血选择性插管,接受表阿霉素(EADM)+顺铂(DDP)+异环磷酰胺(IFO)动脉灌注联合全身化疗。EADM(总量60～90 mg/m2)第1天经导管持续灌注6～8 h;DDP(总量80 mg/m2)分成3～5 d使用,其中第1天的量经动脉导管推注,余量由外周静脉滴注;IFO(总量约5克/m2)分成5 d由外周静脉滴注。每3周为1周期,治疗至少2个周期后观察疗效及毒副反应。结果全部患者均完成了2周期以上的化疗,平均3.2个周期。其中进展2例、稳定2例、部分缓解6例,总有效率60%。Ⅲ+Ⅳ度白细胞、血小板、血红蛋白减少发生率为50%、30%、40%,经相应治疗后均恢复正常。结论动脉灌注联合全身化疗治疗晚期软组织肉瘤近期有效率高,无严重毒性反应,总体耐受性良好,但远期疗效有待观察。
Objective To study short-term therapeutic effect and toxicity of intra-arterial infusion（IA） combined with intra-venous（IV） systemic chemotherapy in advanced soft tissue sarcoma（ASTS）.Method 10 cases with ASTS were catheterized in main feeding artery by right femoral artery puncture technique.All cases received IA infusion combined with IV systemic chemotherapy with combination regimen of epirubicin（EADM）,cisplatin（DDP）,and ifosfamide（IFO）（epirubicin 60-90mg/m2,IA for 6-8 hours,day 1;cisplatin,80 mg/m2,IA,day 1 and IV,day 2-4;ifosfamide,5.0/m2,IV,day 1-5）.The therapy was repeated every 3 weeks for at least 2 cycles before evaluating the effects and toxicities.Results Ten cases of evaluable patients had completed at least 2 cycles of chemotherapy with an average of 3.2 cycles.The overall response rate was 60%（PD 2 cases,SD 2 cases,and PR 6 cases）.The rates of grade 3/4 neutropenia,thrombocytopenia and anemia were 50%,30%,and 40%,respectively.All abnormalities were rectified after corresponding treatment.Conclusions Intra-arterial infusion combined with systemic chemotherapy regimen has higher response rate in ASTS,with milder toxicity and better tolerance,but long-term effect should be observed further.
