摘要
目的观察加强营养支持对老年性痴呆合并吞咽功能障碍病人综合干预治疗的疗效影响。方法选择合并吞咽功能障碍的老年性痴呆患者48例,随机分成瑞先组(瑞先营养支持)和对照组(普通饮食),采取综合干预治疗后,观察患者的营养状况,指标包括:体重指数(BMI)、测量肱三头肌皮褶厚度(TSF)、上臂中围(MAC)和臂中部肌围(MAMC),以及6个月内发生相关感染的天数及其吞咽功能的改善情况(参考洼田氏饮水试验评价分级)等相关指标变化。结果瑞先组患者在治疗后血红蛋白(Hb)、血清总蛋白(TP)、白蛋白(Alb)、BMI、TSF、MAC、MAMC、淋巴细胞计数和感染天数与治疗前比较,差异均有统计学意义(t分别=5.97、13.34、15.41、7.40、3.71、4.13、3.73、4.53、10.00,P均<0.05);对照组治疗后的TP、Alb、淋巴细胞计数、感染天数与治疗前比较,差异均有统计学意义(t分别=2.42、6.85、2.56、6.23,P均<0.05)。瑞先组和对照组治疗后洼田氏饮水试验分级指标与治疗前比较,差异均有统计学意义(χ2=21.89、18.32,P均<0.05)。结论对于合并吞咽功能障碍的老年性痴呆患者,在进行综合干预的基础上,加强营养支持是提高老年痴呆患者吞咽能力,减少相关感染的有效辅助治疗方法。
Objective To evaluate the effects of fresubin energy fiber nutrition on senile dementia patients with deglutition disorder in comprehensive intervention. Methods Forty eight cases of senile dementia patients with deglutition disorder were randomly divided into control group （common diet, 24 cases） and fresubin energy fiber group （fresubin energy fiber nutrition, 24 cases）. All patients were treated by comprehensive intervention therapy. The deglutition function （water-drinking test）, body mass index （BMI）, triceps skin fold （TSF）, mid-arm circumference （MAC）, mid-arm muscle circumference （MAMC）, hemoglobin （Hb）, total protein （TP）, albumin （Alb）, and the days of infection within 6 months were measured. Results After the comprehensive intervention therapy, the levels of Hb, TP, and Alb, BMI, TSF, MAC, MAMC, and the number of lymphocytes were all significantly increased （t=5.97, 13.34, 15.41, 7.40, 3.71, 4.13, 3.73, 4.53, P〈0.05）, and the infection days were significantly decreased （t=10.00, P〈0.05） in the fresubin energy fiber nutrition group. After the comprehensive therapy, the levels of TP, Alb and lymphocytes were significantly increased （t=2.42, 6.85, 2.56,P〈0.05）, and the infection days were significantly decreased （t=-6.23, P〈0.05） in the control group. The deglutition function in both groups were improved （X^2=21.89, 18.32, P〈0.05）. Conclusions Based on the comprehensive intervention therapy, fresubin energy fiber nutrition support is an effective auxiliary therapy to improve deglutition function and to reduce infections in senile dementia patients.
