摘要
十二指肠乳头肿瘤发病率低,临床症状不典型,传统治疗采用胰十二指肠切除术,但并发症发生率较高。随着研究的深入,十二指肠乳头肿瘤局部切除术逐渐开展,包括开腹局部切除术、腹腔镜辅助下局部切除术及内镜下局部切除术,越来越多的学者认为其预后较胰十二指肠切除术好,但需严格掌握适应证。本文就十二指肠乳头肿瘤生物学特性、病理、临床诊断及局部切除手术治疗进展综述如下。
Duodenal papillar neoplasm has low incidence and atypical symptoms, and it was treated by pancreaticoduodenectomy （ Whipple operation） formerly. With the development of research, local resection of duodenal papillar tumor has been applied gradually, including open abdominal approach, laparoseopic assisted approach and endoscopic approach. More and more research results support that the local resection can be achieved superior prognosis than Whipple operation for duodenal papillar tumor if indication has been selected strictly. In the article, the biological characteristics, pathological type, advancement of diagnosis and surgical intervention of local resection are reviewed.
