摘要
目的探讨64排螺旋CT三维成像技术在颅底肿瘤外科中的应用价值。方法颅底肿瘤患者109例,包括脑膜瘤40例,神经鞘瘤18例,动脉瘤17例,胆脂瘤8例,垂体瘤22例,血管母细胞瘤4例,术前64排CT三维重建,以容积再现法(VR)及最大面积投影法(MIP)等作图像三维重建处理,模拟手术入路。结果64排螺旋CT三维成像清楚显示了颅底肿瘤的生长部位、形态、大小及其与大血管、颅骨的三维关系,与手术及病理检查吻合。结论64排螺旋CT三维重建可清楚显示肿瘤、血管及其与邻近组织的关系,对颅底肿瘤诊断及鉴别诊断、设计手术入路及方法有着重要价值。
Objective To investingate the clincal value of applying three-dimensional(3D) reconstruction of 64-slice helical CT on diagnosis and surgical treatment the skull base tumor. Methods Consecutive 109 cases with skull base tumors were examined with 64-slice helical CT and 3D reconstruction. Among all the cases,there were reported 40 cases with meningioma, 18 cases with myosehwannoma, 17 cases with aneurysm,8 cases with cholesteatoma, 22 cases with pituitary tumor and 4 cases with hemangioblastomas. The techniques of volume rendering (VR) and maximum intensity project (MIP) were used to reconstruct the 3D images in all eases and the approach of operation was simulated. Results 64-slice helical CT clearly demonstrated the location, size and contour of skull base tumors. It displayed the 3D shapes of the neoplasms and their relationship with adjacent vessels and the skull base, which were approximately the same as intra-operative and pathological finding. Conclusion 64-slice helical CT with 3D reconstruction could clearly display the correlations of tumor, blood vessel and the peripheral constructions, h may play an important value in diagnosis, differential diagnosis and preoperative evaluation of operative approach in skull base tumors.
