摘要
目的探讨龙黄合剂体外抑Hp作用,及对Hp阳性十二指肠溃疡的疗效.方法采用纸片扩散法和琼脂稀释法,对龙黄合剂中单味中药及复合制剂进行体外抑Hp实验,并与部分中成药抑Hp效果对比.将128例Hp阳性十二指肠溃疡随机分成龙黄合剂组(A组),雷尼替丁+呋喃唑酮组(B组)及龙黄合剂+呋喃唑酮+雷尼替丁(C组)3组进行治疗,观察各组溃疡愈合率,复发率,Hp根除率,胃窦粘膜病理好转率,药物副反应发生率.结果龙黄合剂有较好体外抑Hp作用.C组Hp根除率为848%,溃疡1a复发率135%,胃窦粘膜炎症病理好转率为653%,副反应发生率为130%,均优于A组(452%,375%,571%,71%)及B组(375%,500%,375%,195%),统计学处理有显著或极显著差异(P<005或001).3组溃疡愈合有效率分别为C组1000%,A组900%,B组905%,统计学处理无显著差异(P>005).结论体外试验证明龙黄合剂对Hp有较好抑制作用,龙黄合剂+雷尼替丁+呋喃唑酮用于Hp阳性的十二指肠溃疡治疗,可提高Hp根除率,减少复发率,副作用小,价格低廉,服用方便,适合国情。
IM To investigate the extrasomatic inhibition of LongHuang mixture (mixture gentianae rheum, MGR) on Helicobacter pylori (Hp) and its clinical effect on Hppositive duodenal ulcer patients.METHODS The paper diffusion and the agar dilution methods were used in the extrasomatic inhibition test of Hp with MGR alone and in combination with other drugs. Duodenal ulcer patients, identified as Hppositive, were randomized into 3 groups, i.e. the MGR group (group A, n=42), the ranitidine + furazolidone group (group B, n=40) and the MGR +ranitidine + furazolidone group (group C, n=46) to observe their Hp eradication rate, ulcer healing rate, pathological improvement rate of the gastric antrum mucosa and the incidences of side effects.RESULTS MGR had a rather better extrasomatic inhibition effect on Hp. In group C the Hp eradication rate was 848%; the pathological improvement rate of the gastric autrum mucosa was 653%, while the incidences of side effects was 130%; all of which were better than those in group A (452%, 375%,571%,71%) and group B (375%,500%,375%,195%). Statistical analysis revealed that there were significant or highly significant difference in the results (P<005 or <001). The ulcer healing rate was 1000% (group C), 900% (group A) and 905% (group B).CONCLUSION MGR has a better inhibition effect on Hp. MGR + ranitidine + furazolidone can improve the eradication rate, reduce ulcer recurrence and side effects. Because of its low cost, MGR suits the China market and worthy of recommendation for clinical use.
