肾小球疾病尿液足细胞排泄的特征

摘要 目的探讨在各种肾小球疾病中尿液足细胞排泄的特征。方法将96例因肾小球疾病行肾穿刺病理活检的患者作为病例组,根据肾脏病理改变,分为肾小球活动性病变组和肾小球非细胞增生性病变组,40例健康志愿者作为对照组,采用免疫酶细胞化学方法,应用小鼠抗人podocalyxin(PCX)单克隆抗体进行标记,计数足细胞,比较分析各组尿液足细胞阳性率及尿液足细胞的平均排泄水平。结果1)96例肾脏疾病患者中,65例尿液足细胞阳性,占67.71%;而40例健康志愿者中尿液足细胞均为阴性,差异均具有非常显著性意义(P<0.01)。2)96例肾脏疾病患者中,肾小球活动性病变组,尿液足细胞检测的阳性率为75.81%,足细胞平均排泄水平为(6.48±8.03)个/20HP;而肾小球非细胞增生性病变组,尿液足细胞检测的阳性率为52.94%,足细胞平均排泄水平为(2.65±3.92)个/20HP,两组比较差异具有显著性意义(P<0.05)。结论在多种类型的原发性或继发性肾小球疾病中均有足细胞的损伤脱落,尿液足细胞的排泄水平与肾小球疾病肾脏病理活动性密切相关。 Objective The aim of this study was to investigate characteristics of urinary podocyte excretion in glomerular disease. Methods 96 renal biopsy patients of glomerular disease as case group, and 40 healthy volunteers as control group were sdudied. The case group divided into glomendar disease activity group and the non-cell proliferative lesions group according to the renal pathological changes. Urinary podocytes were examined by immunocytochemistry methods and makerd by mouse anti-human podocalyxin (PCX) moneelonal antibody. We analyzed the positive rate of podeeyte in each group and urinary podocyte excretion level of the average. Results 1 ) Among 96 cases of patients with renal disease,65 cases(67.71% ) urinary podocytes were positive and none of the 40 healthy volunteers had urinary podocytes. There was statistically difference between two groups ( P 〈0. 01 ). 2)96 cases of patients with renal disease expressed as active glomerular disease. ,the positive rate of detection podeeytes in the urine was 75. 81% and the mean level of urinary excretion of podocytes was 6.48 ± 8.03/20HP;however,the positive rate of detection podoeytes in the urine was 52. 94% and the mean level of urinary excretion of podocytes was 2. 65 ± 3.92/20HP in the group which didn' t expressed as ceU proliferation. There was statistically difference between two groups ( P 〈 0. 05 ). Conclusion Podeeyte injury was detached in various types of primary or secondary glomerular diseases and rinary excretion of podeeytes is related to the activity of pathology of glomerular disease.
