The Effects of Granu1ocyte Colony Stimulating Factor on Neutrophilic Alkaline Phosphatase Activity and the Neutrophil Morphlogy

摘要 目的探讨粒细胞集落刺激因子(granu1ocyte colony—stimulating factor,G-CSF)对中性粒细胞碱性磷酸酶(neutrophilic alkaline phosphatase,NAP)及形态学的作用。方法选择化疗中使用G-CSF无合并感染的肿瘤患者为G-CSF组,具有细菌感染阳性指征的患者为感染组,另外选择100例正常体检者作为对照组。分别对三组的血涂片进行NAP染色,并作阳性率、积分计数,NAP阳性指数行单因素方差分析实现两两比对分析,同时对三组进行瑞氏染色观察中性粒细胞形态。结果100例体检者NAP阳性指数95%的医学参考区间为13~127。G-CSF组、感染组和对照组的NAP阳性积分分别为290±56、245±55和68±27,三组经单因素方差分析,两两间比较差异均有统计学意义(P<0.01);阳性率分别为(95%±5%)、(89%±8%)和(51%±18%),两两之间比较差异均有统计学意义(P<0.01)。形态学方面,感染组与对照组差异不明显,感染组、对照组分别与G-CSF组的差异较为明显。结论G-CSF可致中性粒细胞NAP阳性率、积分明显升高,并且明显高于感染所致NAP阳性率和积分的升高。在形态上,G-CSF可使中性粒细胞胞浆颗粒明显增多增粗,同时易见空泡变性及幼稚中性粒细胞。 Objective To study the effect of granulocyte colony stimulating factor (G-CSF) playing on the neutrophilic alkaline phosphatase (NAP) activity and morphological characteristics of neutrophils. Methods The malignant tumor patients treated by G-CSF were chosen as G-CSF group, and those with bacterial infection were chosen as infection group. The NAP scores from one hundred healthy individuals were used as the 95% medical reference range. Ruled out the persons who were out of the range, and the remains were selected as control group. The blood smears of three groups were used for the detection of NAP by staining to obtain the positive rate and NAP scores. The one-way ANOVA was adopted in analyzing the positive index. And the neutrophils were stained with Wright stain to reveal the morphological characteristics. Results The 95% medical reference range of the positive rate, which was determined from one hundred health-screening persons, is 13-127. The NAP positive scores of the G-CSF group, infection group and control group were 290+56, 245+55 and 68-27, respectively. The multiple comparisons among the three groups showed that there were significant differences among the three groups (P〈0.01), with the positive rates of (95%-5%), (89%-8%) and (51%-18%); and there are significant differences in group-by-group comparisons (P〈0.01). In morphology, there was no obvious difference between infection group and control group, but a significant difference was presented between control group and G-CSF group. Conclusion G-CSF up regulates the activity of NAP obviously, even exceed the increase caused by bacterial infection. In morphology, vacuolization and thickening of particles in neutrophil can be observed. And, immature neutrephils were not unusual to be found in blood smear.
