摘要
目的 研究血清涎酸 (SA)对贲门癌诊断及监测的临床价值。方法采用 Honda S改良的荧光测定法 ,对 115例贲门癌 ,85例良性疾病患者和 10 3例健康人进行血清 SA测定。结果 贲门癌组患者血清 SA含量显著高于良性疾病组和健康人组 (P<0 .0 1) ;以血清 SA水平大于 1.92×10 - 3 mol/L)为检测阳性 ,则对贲门癌诊断的敏感度、特异度、准确度依次为 83.5 %、89.4%、86 .0 % ;不同组织类型和临床分期的贲门癌患者血清 SA间也明显差异 ,血清 SA水平与肿瘤的分化程度和临床分期成正比 ;贲门癌根治术 4周后的血清 SA水平显著低于术前 (P<0 .0 1) ;贲门癌复发或有转移 ,则血清 SA再次明显升高或持续在高水平。结论 测定血清 SA对贲门癌诊断、疗效观察及术后监测具有显著的临床价值。
Objective:To evaluate the clinical significance of serum sialic acid (SA) measurement in diagnosing and monitoring cardia cancer.Methods:Serum SA concentrations of 115 cardia cancer patients,85 patients with benign disease and 103 normal persons were measured by Honda S modified fluorescence method.Results:The serum SA level in cardia cancer patient group was markedly higher than that in the other two groups(P<0.01).When the serum SA level over 1.92×10 -3 mol/L was taken to be a cardia cancer positive index,the sensitivity,specificity and accuracy of cardia cancer diagnosis were 83.5%,89.4% and 86.0%,respectively.The serum SA levels of cardia cancer patients in various tissue types and clinical pathological stages were obviously different and positively related to the degree of the cancer development.The serum SA level of patients 4 weeks after radical operation was markely lower than that before operation (P<0.01),but the serum SA level would be rebounded to a higher level when relapse or metastasis of cancer happened. Conclusion:Measurement of serum SA reveals marked significance in diagnosing and monitoring cardia cancer in patients before or after operation.
