前路减压内固定术治疗胸腰椎骨折 被引量:1

Anterior decompression and internal fixation in the treatment of thoracolumbar spinal fracture.

摘要 目的探讨前路减压植骨内固定手术治疗胸腰椎骨折的疗效,分析前路手术优缺点。方法我院于2006年1月至2008年9月采用前路减压加椎间植骨钛板内固定治疗胸腰椎骨折27例,随访观察其功能效果,分析前路手术的适应证及优缺点。结果本组所有病例脊髓均获得有效减压,术后获随访6-28个月,平均14.2个月。术后出现的神经功能改善情况:24例不全截瘫患者中均有Ⅰ-Ⅲ级恢复;1例全瘫患者未有任何恢复;2例无神经损害者术后功能良好。随访x线见植骨均已融合,无神经、血管、输尿管损伤并发症。结论前路减压植骨内固定术减压彻底,可重建脊柱的稳定性,神经功能恢复良好,是治疗胸腰椎骨折的一种有效的方法,但是要注意防范并发症。 Objective To investigate the curative effect and analyze the advantage and disadvantage of anterior decompression and internal fixation in the treatment of thoracolumhar spinal fracture. Methods January 2006 to September 2008, 27 cases with thoracolumbar spinal fracture treated by anterior decompression and internal fixation were reviewed retrospectively according to the advantage and disadvantage, indications, operative approaches and options of internal fixation. Results All patients with spinal cords injured were decompressed effectively in the durations of follow - up ranged from 6 months to 28 months ( average of 14.5 months). 24 patients with non - complete paraplegia recovery were achieved 1 to 3 degree after operation, 1 patient was complete paraplegia, 2 patients was non - nervous lesion and recovered fully after operation. The grafted bone was fusion as showed by X - ray film and had non - complications of nervous lesion, blood vessel and ureter. Conclusion Anterior decompression and internal fixation is an effective approach to treat thoracolumbar spinal fractures ,which it can decompress the vertebral canal, rebuild spinal stability, and revert spinal function. Meanwhile, it' s necessary to take measures to prevent complications.
