摘要
目的分析支气管结核误诊原因并探讨其治疗。方法回顾本院近3年来经纤支镜作病原学和/或病理学确诊的溃疡坏死型支气管结核108例进行临床分析。结果诊断支气管结核溃疡坏死型108例,男3例、女105例。肺部有病灶者76例,无病灶者32例。术前临床诊断支气管结核72例,术中诊断36例。术前痰菌阳性83例,阴性25例。通过全身治疗加经纤支镜局部治疗有效率100%、治愈率95%。结论经纤支镜检可早期诊断支气管结核,避免误诊、漏诊,经纤支镜局部治疗可提高疗效,减少并发症,有效避免外科手术治疗。
Objective To analyze the reasons of misdiagnosis of bronchus intimal tuberculosis, and to discuss its therapy. Methods The study reviewed and analyzed 108 patients of our hospital in reeent 3 years, who were diagnosed definitely as ulcerous and necrotie bronehus intimal tubereulosis by bronehofibroseopy on etiologic and/or pathological examination. Results In the 108 patients of ulcerous and necrotic bronchus intimal tuberculosis, there were 105 women and 5 men. 76 patients had pulmonary focus but 32 patients had no focus. 72 patents had been diagnosed clearly in clinic before bronehofibroseopy and 36 were diagnosed through bronchofibroseopy. Before operations 83 were sputum - smear positive and 25 were negative. The effective rate of therapy with both systemic anti-tuberculosis therapy and local bronchofibroseopy therapy was 100%, and the cure rate was 95%. Conclusion Bronehofibroscopy can diagnose bronchus intireal tuberculosis early. It can avoid misdiagnosis and missed diagnosis. The local therapy by bronehofibraseopy can improve the curative effect, decrease complication, and can avoid surgery operation effectively.
